The investigation report shows that at 21:59 on November 24, 2023, during the concrete pouring process in the No. 2 engine room of the raw material coal shed of the coal blending system of the Yongxin Railway Special Line Container Station Construction Project of Shanxi Yongxin Tonghai Railway Logistics Co., Ltd. A collapse accident of the formwork support system occurred, killing 7 people and causing direct economic losses of approximately 19.4671 million yuan. The Shanxi Provincial Work Safety Commission supervised the accident.
After investigation, it was determined that the major collapse accident of No. 2 engine room "11·24" at the Yongxin Railway Special Line Container Station of Linfen Anze Shanxi Yongxin Tonghai Railway Logistics Co., Ltd. was caused by a lack of construction safety management and the formwork support system carried Large collapse production safety accidents caused by insufficient strength and stability.
The direct cause of the accident was: the vertical and horizontal distance between the vertical poles of the formwork support system was too large, and the axial stress of the vertical poles seriously exceeded the standard design value of the steel pipe; the horizontal pole step distance of the formwork support system was too large, and the frame was not equipped with horizontal scissor braces, making the frame too high The width ratio does not meet the specification requirements and there are no wall-connecting parts throughout the entire height. There is no effective connection between the frame body and the frame structure, which reduces the overall stability of the scaffolding. The above unfavorable factors lead to the collapse of the formwork support system during the concrete pouring process.
