Community Health Management Expert Guidelines

  (first edition)

These guidelines are used to standardize and guide community health service institutions (including centers and stations) on the health management of new coronavirus positive infections (including nucleic acid positive or antigen positive, hereinafter referred to as new coronavirus positive infections) and their co-residents within their jurisdiction Serve.

The health administrative departments of each district can formulate relevant work guidelines in their region according to the basic requirements of these guidelines and in light of the actual situation.

  1. Information management

When the new coronavirus positive infection is included in the management, it is necessary to provide a positive nucleic acid or antigen positive test result.

Establish resident health records for people who are positively infected with the new coronavirus and their co-residents, understand their daily health status, and fill in relevant information on new coronavirus infection as required.

Establish a work account to grasp the basic information such as the name, contact information, home address, and co-residents of the new crown virus positive infected person.

Special attention should be paid to orphans, children, pregnant women, dialysis patients and special patients with serious underlying diseases.

  2. Health assessment

  1. Assessors

Asymptomatic infections and mild patients should be evaluated by general practitioners from community health service institutions.

For severe mental illness, pregnant women and children should be evaluated by specialists (gynecology, pediatrics) or health care physicians.

  2. Assessment Guidance

  (1) Asymptomatic infection

Those who have been tested positive but have no relevant clinical symptoms (such as fever, cough, sore throat and other self-perceived or clinically identifiable symptoms and signs), can be given health education, including how to ventilate, disinfect, hand hygiene and other health education, as well as daily life TCM characteristic guidance and intervention such as adjustment of intake, diet guidance, exercise guidance, emotion adjustment, prevention of traditional Chinese medicine, etc.

  (2) Mild/common patients

With fever, respiratory symptoms and other symptoms, no dyspnea, chest tightness and other performance.

Mild/common patients generally do not need special treatment, and general symptomatic treatment can be given when symptoms such as fever and respiratory tract occur.

It is recommended to stay in bed at the early stage of infection, drink plenty of water, and ensure adequate energy and nutritional intake.

If fever symptoms occur, necessary antipyretic drugs can be given, such as ibuprofen and acetaminophen, etc., according to the patient's condition.

For cough and sputum symptoms, relevant drugs can be given to relieve symptoms.

Patients with chronic diseases need to take daily chronic disease maintenance medicines on time. If there is tension and anxiety, psychological counseling should be strengthened. If necessary, drug treatment should be supplemented under the guidance of a doctor and Chinese medicine intervention guidance can be given.

For children: In addition to symptomatic treatment, children's massage, acupoint massage, music therapy, etc. can be taken appropriately to help stabilize emotions and protect mental health.

For pregnant women: pay attention to the physical and mental state at any time, and provide consultation and guidance on diet, emotional psychology, and professional doctors to give guidance when necessary.

For the elderly: In addition to the symptoms of the new crown, pay attention to the underlying diseases of the elderly, and strengthen intervention and health guidance for basic diseases, such as guiding chronic disease medication, eating habits, exercise, etc.

For patients with mental illness: pay attention to the mental state at any time, and provide targeted psychological counseling and symptomatic treatment by professional doctors.

  (3) Severe/critical patients

If the respiratory rate is greater than 30 breaths/min, there is hypoxemia, and in severe cases, respiratory failure and shock may occur.

The critically ill persons with mainly symptoms of new coronary pneumonia were transferred to designated hospitals in time.

  (4) Residents

Carry out health education for people who live with people who are positive for the new coronavirus, including how to avoid simultaneous and spatial exposure, ventilation, disinfection, hand hygiene, daily life adjustments, dietary guidance, exercise guidance, emotional adjustments, and prevention of traditional Chinese medicine and other infection prevention measures.

  3. Propaganda notice

Through paper or electronic notices, etc., inform the self-health management requirements of the new coronavirus positive infection, push the antigen self-test video, user manual, health and epidemic prevention knowledge, guide the new coronavirus positive infection to carry out self-test and symptom monitoring in a standardized manner, and improve Epidemic prevention skills.

Inform those who are positively infected with the new coronavirus the daily medical protection method, medical treatment process, and related work contacts and contact information.

  4. Health management

Through the combination of online and offline methods, it provides services such as health follow-up and remote diagnosis and treatment for people who are positive for the new coronavirus.

Carry out health monitoring, ask and collect the body temperature, respiratory symptoms, mental state and other information of the new crown positive infected persons every other day, whether they have dyspnea, chest tightness and other symptoms, until the new crown virus positive infected persons self-test antigen test or nucleic acid test negative.

If necessary, we can provide door-to-door visits, assess health conditions, and deal with them in a timely manner according to the assessment conditions.

Simultaneously do a good job in the health monitoring of the people living with the new crown virus positive infection, and guide them to seek medical treatment in time if abnormalities are found.

  5. Referral guidance

When a person with a positive infection of the new coronavirus develops a critical illness such as persistent fever and dyspnea, the patient or family member should be instructed to go to the nearest medical institution for treatment by self-driving or calling 120 ambulance.

For non-emergency and critically ill patients who seek medical treatment for special diseases such as dialysis, they are guided to give priority to designated hospitals in the jurisdiction, and assist in referrals when necessary.

  6. Case closing management

After the self-test antigen test or nucleic acid test negative of the new coronavirus positive infection, the health management of the patient will be terminated, and it will be included in the scope of routine health management of community health services.
