A pioneer in the treatment of skin cancer using Mohs Surgery, a master of biostimulation (induction of collagen production naturally) and a hair specialist, José María Ricart was designated by 'Forbes Spain' as one of the best dermatologists in our country (the waiting list to get a place in its very busy schedule confirms this).

The founder of the Ricart Medical Institute, head of the Dermatology Service at the Quironsalud Valencia Hospital and medical director of IMR at Ruber Internacional Paseo de la Habana (Madrid,) treats an average of 50,000 patients a year at his clinics in Madrid, Valencia, Alicante and Torrent.

Those who work with you, ensure that you are tireless.

Is it true that he consults every day from nine to nine, without a break for lunch? Actually, I prefer to start at 8 am.

I train every morning from six to seven and then I go to consult.

God helps those who get up early and, for me, working every day is an illusion.

I wake up wanting to attend my patients because, in addition to being a

very rewarding experience

, the truth is that I had a great time during my working day.

It is an active job, with many incentives and many joys.

I spend the day surrounded by people who love me.

Sometimes everything doesn't turn out as perfect as you'd like, but 95% of the time it does, it's worth it.

So, every day I go to enjoy myself instead of working, for that reason working more or less hours is totally irrelevant and has no merit.

As Confucius said: "Choose a job you like and you will never have to work a day in your life."

There are many people who work as much or more than me, I think what really stands out is that they don't stop to eat.

It is scientifically proven and there are therefore many published works that confirm that

Intermittent fasting is beneficial for delaying aging

, it allows us to be alert for more hours a day, it allows us to reduce inflammation and oxidation in our body.

That is why I am a faithful follower of intermittent fasting and I practice it from Monday to Friday because it makes me feel very good.

I don't do it on the weekend.

But, be careful because not everyone should do it, it is important that there is medical supervision if someone decides to do it. You come to Madrid twice a month and the waiting list to get a place in your schedule is endless.

What is the secret? As is logical, having a consultation two or three times a month in Madrid, I always have a full agenda.

But I am lucky to have a great team.

The secret of our success is that our group, IMR, has grown on very solid pillars:

constant

research ;

ongoing training

for all medical staff and equipment;

work and care protocols for

the different needs of patients following the most up-to-date and efficient standards;

and the use of

cutting-edge technology

applied to all our specialties.

I am extremely lucky to have a magnificent team of medical professionals who ensure that patient care and treatments will always be of the highest quality, regardless of whether I am in consultation that day or not. What do you think they find in you patients?Patients want you to solve their problems, advise them, guide them.

For me, quality is the total satisfaction of the patient, I like to give them what they need, no more, no less.

Solutions.

To me, patients are like family.

You have to treat them with affection, proximity and sincerity.

And you end up making friends with many of them.

I think they end up finding their trusted doctor in me and that, honestly, is very gratifying.

Dermatology has experienced a real revolution in recent years.

How do you interpret it?

In your opinion, what are the main advances? Dermatology is a very broad medical specialty, with many areas of activity, dealing with autoimmune diseases such as lupus, skin tumors such as melanoma or carcinoma, pediatric dermatology, STD, infectious skin diseases such as monkeypox, or parasitic, such as scabies, trichology (hair diseases) or aesthetic and laser dermatology to highlight a few.

Of all these fields, I would emphasize as the most relevant scientific advances the

Of all these fields, I would emphasize as the most relevant scientific advances the

new therapies for the treatment of skin tumors or autoimmune diseases such

as psoriasis.

What is also undeniable is the progress at a social level that has made

cosmetic dermatology and trichology available

to the vast majority of the population, and the growing interest in skin and hair health, this has meant in recent years a revolution in treatments, in techniques and in

the way of approaching rejuvenation in an integral way.

As you point out, dermatology has become one of the pillars of preventive medicine and healthy ageing.

What will be, in your opinion, the trends for the coming years?

Will we manage to age better and combat the ravages of time in a more natural and healthy way? The world is changing rapidly in terms of health and healthy ageing.

We begin to see the brushstrokes of a new era, that of

predictive medicine

, which goes one step further than preventive medicine.

But for this medicine to be able to globalize, it is necessary for people and professionals to be aware and I think that the time has not yet come.

In the United States, there is already a current of aging management as a form of proactive prevention, but in Spain there are very few specialized centers.

We already know many of the mechanisms of aging, but

predictive medicine allows us to know the particular pattern of aging of each one

, and shows us the particular areas or weak points of a person in which it makes more sense to work to slow down our aging.

We already have supplements and health optimization treatments to modulate the way we celebrate our birthdays, improving quality of life and preventing diseases.

And, currently, research and studies are being carried out to ensure that the human body, despite the passing of the years,

continues to create muscle mass, to produce brown fat and to lengthen telomeres

.

As you can see we are not talking about chimeras.

The

introduction into our body of vectors with genetic information that modifies the reading of our genome

is capable of making these changes.

We send information to the command center of our cells so that they execute different orders than those foreseen.

It is a

mechanism similar to the one we have used for the coronavirus vaccine

that has become so famous.

We are talking about the present.

In the coming decades, these treatments can be applied in consultation.

The current objective is to reach these advances in the best conditions, so as not to miss the boat.

At the moment, for all those who cannot access this medicine, it is important to remember that the foundations of any healthy longevity plan are available to everyone:

avoid toxic

substances (tobacco, alcohol),

exercise and take care of your diet. .

There is much more that can be assessed and done today, but these measures are basic and irreplaceable for many other treatments that we do.

With recent advances, I am convinced that

the human being, in 2060, will easily live up to 100 years

, yes, as long as we do not burden the world with stupid wars. Sounds exciting... It is .

Biogerontology studies the aging process and how to stop it, not to live 200 years, but to

gain quality of life and delay diseases in the last third of our lives

.

In recent years, great advances are being made in the way of measuring true biological age such as the

measurement of telomere length.

, DNA methylation clocks, immunological or metabolic age.

They are very important to be able to analyze the effectiveness of treatments.

In this way, basic pathways have been discovered for the maintenance of self-regeneration, such as the

sirtuin enzymes and the maintenance of NAD levels

(derived from vitamin B3, also known as niacin, niacinamide or nicotinamide, it is used in more than 500 reactions and has a crucial role in

metabolism, cellular aging, DNA repair, and gene expression

).

Also, the importance of reducing the burden of aged (senescent) cells and "cellular junk" by intermittent fasting and senolytics to activate autophagy has been revealed.

In an almost science fiction field, the discovery of

the regenerative power of the WNT pathway or the Yamanaka factors is tremendously promising

, capable of rejuvenating our cells.

Recently, a study was published in 'Nature', carried out by Dr. Izpisua's group, in which human skin was rejuvenated by 30 years with these treatments.

The near future, as you can see, is fascinating, but today we already have enough reliable assessment and treatment tools to get to it in the best conditions. Suddenly, everyone is talking about collagen.

Does it really play such a crucial role in how we age?

Why? As we age, scientific studies have shown that

we go from being a grape to a raisin

.

No other example is more prescient.

Gradually, bone resorption occurs and the fat and skin become thinner.

Collagen plays an important, though not unique, role in this process.

The trabeculae of the septa that make up the fat, the fibrous septa, and the dermis contain significant amounts of collagen.

If we add to this the fact that the latest advances have made tools available to clinicians to restore this collagen and that pharmaceutical companies invest huge amounts in advertising and in the training of doctors, we have the perfect cocktail for collagen to jump to the fore as sonorously.

In any case, the scientific part is always ahead and the works have made clear the relevant role of collagen.

What can we do to slow down its destruction? Not using tobacco and protecting ourselves from the sun, in addition to strictly following all the instructions that I have indicated above to age in a healthy way. You are a true master of biostimulation.

Can you explain to us what it is, what its benefits are and how it is done? The biostimulation of the different components of the skin and its adjacent tissues allows, through different procedures - such as

injection

of platelet-rich plasma, hyaluronic acid, calcium hydroxyapatite, polylactic acid or polycaprolactone, and the use of different

technologies

, such as high-intensity ultrasound or radiofrequency, among others- activate the mechanisms that start

tissue regeneration and collagen formation

.

This type of rejuvenation is gaining great popularity because it is based on regenerating and restoring our tissues, avoiding the facial deformities that were obtained years ago with treatments such as permanent fillers or injections of huge amounts of botulinum toxin.

With biostimulation we manage to restore our beauty.

We are ourselves, but rejuvenated, and that is what 99% of my patients are currently looking for.

I must clarify that, although I have named many procedures, not all are equally effective or safe.

Using one or the other will depend on the needs of each skin.

It is vital that

these treatments are always carried out by professionals

with advanced knowledge. Beauty rituals are increasingly complicated.

How do we take care of our skin?

What products are really essential, day and night? I believe that the really important thing is to choose

products that are suitable for each skin

.

That is the essence of success.

For this, it is essential to go to a dermatologist who will do an analysis, if possible with suitable technology to carry out a detailed study of your skin and indicate a complete plan to take care of it.

We must always bear in mind the importance of making

daily use of photoprotection

, adapting it to our skin type, this is, for me, the really essential product.

We, at IMR, have a unit within Aesthetic Dermatology led by Dr. Rodríguez-Villa with the support of Dr. Sardá, who is specifically specialized in skin quality.

In it, through Adonia technology, we evaluate the skin of each patient and develop a specific plan to improve four major aspects: skin firmnes, skin tone evenness, skin glow, and skin surface evenness. What most influences our way of aging: the genetics or lifestyle? In recent years, scientific studies are showing that we are not captives of our genome.

There is something above our genome, it is called the

epigenome or epigenetics

, that is, the part that is read from the message of our genetic code.

It is as if our genome were a piano with all its keys and the epigenome is the melody that we play on that piano

.

As we interact with the environment, we will have 'Swan Lake' or 'Vivaldi's Four Seasons'.

So, the environment has a decisive influence on the expression of our genes and therefore the relevance of lifestyle, because it has a fundamental role in the way we age.

Some authors point out that

the relationship would be 50/50 and others 30/70

.

I must also tell you that

the best of genetics are spoiled by an unhealthy lifestyle.

So the reading is that we can and must do our part by taking care of ourselves so that our body allows us to enjoy each stage of this path that is life, I'm not talking about looking good;

I am talking about being healthy and physically and mentally active, that is also beauty. What are the main enemies of healthy skin? Tobacco, sun, diet low in antioxidants and environmental pollution. What do we eat and drink? Indeed, high cholesterol or triglycerides, excess alcohol, etc.

They have a direct reflection on our skin.

In addition, there are specific pathologies, such as rosacea, acne or dermatitis, which have been shown to be connected to alterations in the microbiota.

A

good nutrition is a foundation on which good practices are built to slow down our aging

It is very important to include fruits and vegetables, olive oil, nuts and seeds in our diet due to their high content of antioxidants and/or vitamins A, C and E. Is nutricosmetics useful for anything?

What does our skin need? Nutricosmetics came into our lives a few years ago and, as happened before in the US, it has become a huge business in which pharmaceutical companies invest a lot of money in search of new active substances.

This has been very beneficial because it has led to scientific studies that certify the

substances that are really active and effective in the treatment of skin diseases and in the prevention of aging,

among other.

If I had to highlight the most important ones for me, I would name Polipodium leucotomos (skin cell protection from the Dermmia laboratory) and its prevention in skin cancer.

Also oral antioxidants, such as resveratrol, astragalus (rejuvenates-T from the Dermmia laboratory). Is it true that there are no better beauty treatments than happiness and sleeping well? Being happy and having a good rest are two great recommendations at all levels , but unfortunately I don't have the possibility to prescribe happiness to my patients.

However, I can help you look good, blur the passing of the years, remove the tired look from your face, naturally modify any feature you don't like, etc.
