The Metropolitan Police Department sent documents to the two on suspicion of being fatal due to work-related negligence, saying that the mistaken prescription of morphine, which is 100 times the required amount, by a doctor in the clinic and a pharmacist in the dispensing pharmacy led to death.

Last February, a 93-year-old man who lived in Tokyo complained of suffocation and visited the "Musashi Kokubunji Park Clinic" in Kokubunji City.



The man was prescribed oral morphine to relieve his symptoms, but his condition suddenly changed and he died of morphine poisoning a week later, according to an interview with an investigator. ..



An investigation by the Metropolitan Police Department revealed that a doctor in his 40s at the clinic had mistyped an electronic medical record and was prescribing 100 times the required amount of morphine.



In addition, a pharmacist in his 60s at a nearby dispensing pharmacy also dispensed medicines according to the prescription.



The Metropolitan Police Department sent documents on suspicion of accidental fatalities in business, saying that the doctor and pharmacist's failure to confirm enough led to the death.



It means that they all admit the suspicion to any questioning.



The clinic where the doctor works says, "I will not comment" on the interview.
