Beijing Anzhen Hospital has the largest aortic surgery center in China, with more than 1,000 aortic surgeries per year.
These are mostly emergency surgeries, and heart aortic dissection has a high mortality rate, increasing by one percent for every hour of delay.
On the evening of the 28th day of the twelfth lunar month, Uncle Wang, a 60-year-old patient, came to Anzhen Hospital for treatment. He suffered from acute aortic type A dissection. The doctors of Mile Nan's team immediately arranged a series of examinations and preoperative preparations for him.
In order to buy more time for the patient, before 6 am on the 29th of the twelfth lunar month, Mile Nan, who was supposed to be on vacation at home, went out. He arrived at the unit after 7 o'clock, and the operation officially started at 8 o'clock.
"If the patient doesn't do it, the risk of surgery is very high. Once the blood vessel ruptures, the consequences will be disastrous. Moreover, Uncle Wang also suffers from cerebral infarction, glaucoma, blindness in both eyes, right common carotid artery occlusion and other combined symptoms. The situation is very complicated and the risk of surgery is also high. "
In order to perform this major operation, more than 10 medical staff, from anesthesiologists to nurses, came together to escort the patients. Mile Nan was the chief surgeon.
After thoracotomy, Mile Nan found that the patient's preoperative condition progressed very quickly. There was no pericardial effusion during the preoperative CT the night before, but after opening, he found that there was a lot of pericardial effusion. "This shows that the blood vessels have begun to rupture. The pulsation of the aorta may become larger and larger, and if it is delayed, the patient may have no chance."
Replacing the ruptured aorta, reconstructing the carotid artery, suturing... The tense operation was carried out in an orderly manner for 7 hours. The operation was successfully completed at 3 pm. Mile Nan finally breathed a sigh of relief and walked out of the operating room.
不吃不喝站了7个小时，看到还有单位食堂中午发的饺子，饥肠辘辘的里程楠草草地应付了几口。还没等他缓过来，另一位主动脉夹层患者病情突然发生变化，胸痛难耐，经过CT检查，发现患者心脏降主动脉夹层突然破裂，胸腔大量积液。情况紧急，里程楠当即决定要连夜给患者做介入治疗，放心脏支架。
这台介入手术持续到凌晨。放完支架，里程楠看了一眼时间，刚好是大年初一零点08分。结束手术回到家里时已经是凌晨1点多，“家人都还没睡，特意在守岁等我回去团圆，我那4岁的儿子就是不睡觉，一定要等我回家给我拜年。虽然忙了一天，但挽救了两条生命，我还是很快乐的。”里程楠说。
睡了没几个小时，大年初一早上，里程楠又回到病房。“干我们这行，只有亲眼看到病人的情况心里才能踏实，要是因为过年放假不管病人而耽误了病情，再想挽回会费很多周折，而且对病人也是不负责。作为医生，救人是第一位的，现在两个病人都在恢复中。”
每逢过年过节，主动脉外科中心都有手术要做，且病人数量往往比平时更多。“因为一到假期人们就会敞开了吃饭、喝酒、熬夜，血压控制得不好，心脏主动脉夹层发病率很高，希望大家过年的时候要注意休息，科学饮食，控制好血压，尤其是老年人。”里程楠说。
从2002年大学毕业参加工作，里程楠从医已整整20年，已连续10个春节没能回老家吉林长春过年。“工作总是很繁忙，可病人的生命耽误不得。”里程楠说，整个春节假期，主动脉外科中心医生们的手机24小时开机，随叫随到，只要患者有手术指征，他们就会立刻回到手术台。
