At 3:50:56 on March 25, 2021, a large coal and gas outburst occurred at the tunneling face of the 15210 air intake road of Shanxi Shigang Coal Industry Co., Ltd., causing 4 deaths and direct economic losses of 13 million yuan.
On March 27, 2021, relevant local authorities established the "3.25" Large Coal and Gas Outburst Investigation Team of Shanxi Shigang Coal Industry Co., Ltd. to investigate the accident.
The investigation report showed that the direct cause of the accident: the existence of small faults in the working face of the 15210 air intake roadway, the development of structural coal, the superposition of multiple stresses, the risk of coal and gas outburst increased sharply; the pre-drainage time is short, the gas drainage is not up to standard, regional and local prevention Outburst measures failed to eliminate the outburst hazard of the coal body in front of the work; the effect of local outburst prevention measures was tested and the drill holes failed to effectively identify the outburst hazard in the working face; the coal and gas outburst induced by the comprehensive excavator cut the coal and caused the operators to lack oxygen Death by asphyxiation.
Indirect reasons: failure of the enterprise's anti-outbreak responsibilities and loss of control of the safety management; failure of the enterprise to perform the main responsibilities of safety production and safety management loopholes; the local people's government and its safety supervision department are not strict in performing safety supervision responsibilities.
After investigation, it was determined that the accident was a production safety responsibility accident, and 28 related persons responsible were put forward to accountability and handling suggestions: 2 persons were taken by the judicial organs in accordance with the law, 26 persons were recommended by the Jinzhong City Supervisory Committee to be given party discipline and government sanctions, Administrative penalties.
