The "Medical Accident Investigation System," which obliges medical institutions that have suffered fatal accidents to report, reports about 320 cases a year, the smallest number since the system started six years ago, and the bereaved families have reviewed the system. I asked for it and carried out a signing activity.

A total of 15 people, including those who lost their families in a medical accident and supporters, participated in the signing activity held in front of JR Ochanomizu Station on the 7th.

The "Medical Accident Investigation System" started in October 2015 requires all medical institutions to report the cause to the "Japan Medical Safety Investigation Organization", a third-party organization, when a patient dies in a medical accident. It is obligatory.

The Ministry of Health, Labor and Welfare estimates that more than 1,300 fatal accidents occur nationwide annually, but the number of fatal accidents reported under the system in the year to September was 327, the highest in the past six years. It has decreased.

The target of the system is limited to accidents that medical institutions have determined that "death was not expected", and the bereaved families may not have been given the necessary reports and investigations, and if requested by the bereaved family. We are requesting that a third party, etc. conduct an investigation by applying the system.

Masakazu Miyawaki, who lost his two-year-old daughter in a medical accident, said, "There are few reports of medical accidents, and even if a serious accident occurs, it hurts my heart that it does not prevent recurrence. I want you to use the system to help your safety. "
