Three years ago, the 68-year-old president of the company and the 36-year-old captain who died in the accident were sent documents on suspicion of accidental fatal injury.

After that, the Tachikawa branch of the Tokyo Regional Prosecutor's Office conducted an investigation, including hearing from an aviation expert.
