The direct cause of the accident was: Ermugou Coal Mining illegally exploited security coal pillars, which penetrated the 9103 goaf, causing a large amount of gas in the goaf to flow into the coal pillar recovery surface, and the illegal blasting produced an open flame to detonate the gas.
The indirect cause of the accident: Ermugou Coal Mining illegally mines the security pillars, the ventilation management is chaotic, the illegal blasting operation, the coal mine enterprise's management of pyrotechnics is not standardized, the personnel location monitoring system is in vain, the 9102, 9103 stoping faces change the mining technology and fail to follow It is stipulated that the blasting process shall be used in violation of the declaration and coal pillar recovery surface, the labor organization is not standardized, and the hidden danger investigation and safety inspection of coal mining enterprises are mere formalities; Fengyan Group has not implemented the main enterprise safety management responsibilities and strictly implemented the safety inspection system; local party committees, The mine safety supervisors of the government and the supervision department did not conduct the inspections of the Ermugou coal industry, and the safety supervision was not effective. The five-member team of coal mine supervision in Pingyao County failed to perform their duties seriously. The coal mine safety supervision inspection team of Pingyao County did not perform their duties. After the accident, Ermugou Coal Industry did not report the accident within the specified time, which was a late report; the mine also concealed the truth. On November 19, 2019, Ermugou Mine Director Wang Baoxuan, Chief Engineer Liu Huiming, and Safety Deputy Mine Director Wang Anliang , Wang Qiang, Director of Ventilation District and Deputy Technical Mine Manager, and Cheng Peiyao, Deputy Chief of Production Mine, conceal the actual recovery time of the security coal pillars.
