  • Lack of control: de-escalation in Spain: what can be done and when in each phase

The La Paz Hospital in Madrid has registered a possible case of a pregnant patient who has transmitted the SARS-CoV-2 infection, according to the head of the Neonatology Unit of the La Paz Hospital, Adelina Pellicer. Hospital sources point out that although "everything points" that this is a case of vertical intrauterine transmission , "it has not yet been demonstrated."

In fact, the possible vertical transmission of the new coronavirus is being studied worldwide and only a small number of suspected cases have been reported. For this reason, from the hospital they affirm that there are no reasons to generate alarm in a population as sensitive as pregnant women and prefer not to expand information until they have more reliable data, although they report that it is important to report suspected cases to investigate this possible hypothesis. .

Vertical transmission is the transmission of an infection or disease from mother to child during pregnancy, childbirth or lactation. Most cases of mother-to-child transmission occur through close contact between them after birth.

During the pandemic in the hospital, there have been several cases of the disease in newborns due to environmental contact, Pellicer reported to the Efe agency. This case of suspected vertical transmission in pregnancy would have to do with the "sequence of positivity in the newborn".

In order to relate the origin of the infection with vertical intrauterine transmission, the newborn had to be determined with a positive test in the placenta , amniotic fluid, umbilical cord blood or nasopharyngeal and / or oropharyngeal swabs of the newborn through a PCR performed immediately after birth.

"There are some cases reported in the scientific literature," said Pellicer, who, however, pointed out that the symptoms of the coronavirus in babies "seem to be of little relevance" and its evolution does not reach the degrees of severity seen in other populations.

Yes, there have been cases of newborns who have had to be admitted to the hospital after being discharged and infected with SARS-CoV-2 and receiving specific treatment, but the evolution has been very favorable, he said.

During these months, in La Paz, all children born to mothers who had tested positive for Covid-19 or who were under investigation have undergone PCR. "Until it was confirmed whether the newborn was positive or not, we kept them in a specific area to avoid contact with other babies of negative or non-sick mothers."

Currently, all mothers who give birth to the hospital are screened to confirm whether they are infected or not, and the percentage of positive cases has decreased markedly in recent days.

In cases where babies need treatment, Pellicer notes that hydroxychloroquine is used in defined doses and for the duration of treatment established.
