The Spanish Society of Family and Community Medicine (semFYC) has estimated at 900,000 the number of cases with a clinical picture of Covid-19 that have been attended in Spain, and which have not been mostly referred to hospitals.

This is indicated by the results of a survey carried out during the week of March 23 to 27 among 1,063 specialists in Family and Community Medicine who practice in Primary Care. According to this scientific society, it can be deduced from the figures that Primary Care professionals monitor more than 900,000 patients with a probable clinical picture of Covid-19 with mild symptoms and without confirmation .

The projections of the survey data put the confirmed cases as of March 27 at 65,149, a figure very similar to the 64,059 that were published then, which "would give validity to the million minor cases registered," argues the president of semFYC, Salvador. Tranche .

"More than 900,000 cases with mild respiratory symptoms have been treated in Primary Care; of these, only a minimal part is arriving at the hospital , which allows the system to be de-stressed and the hospital units to focus on care for the most serious, thus as in intensive care, as planned, "he reiterates.

Another aspect that attracts the most attention in the survey refers to face-to-face consultations. Although 36.41% of the patients attended in person attended the consultation for reasons related to respiratory symptoms that could be linked to Covid-19, the remaining 63.5% did so for different and indemorable reasons. The total of these face-to-face consultations for non-respiratory issues would be around 867,000, if the data obtained from 1,063 family doctors is projected on the total number of PC physicians.

Activity in these face-to-face respiratory consultations would have involved between 3 and 4 patients per day on average per family doctor, which in data exploitation up to 29,089 family doctors would have meant 494,462 consultations in one week throughout Spain. Regarding the configuration of healthcare activities, a large part of these activities were carried out via telematic means, essentially by telephone.

"Although many professionals have begun promoting video calls with their patients to try to improve the quality of the clinical interview and the results of the same," says Tranche, who notes that "Covid-19 is taking a leap towards real telemedicine. and a very rational use of resources by patients with minor symptoms. "

Quantitatively speaking, this telematic activity may have involved more than 4 million telephone calls made by the group of primary care family doctors during the week analyzed in the survey. This is a data that is extrapolated from the 146,856 telephone calls confirmed by the 1,063 doctors who participated in the survey. The averages advance that each family doctor can be making between 27 and 28 calls a day.
