Every evening, at 7 p.m., the general direction of health reveals the number of cases of coronavirus in France. Does that really make sense since we don't test the entire population?

This figure is only the tip of the iceberg. Because we limit the number of tests to patients who report themselves and those who carry comorbid factors [who present another pathology in addition to suspicion of coronavirus]. So there is a very large part of the population who is sick or likely to be sick and who is not tested. Look at retirement homes or prisons: we don't do tests. If we did, we would find positive cases.

So it doesn't make sense?

Yes. By default, that makes sense. This gives an idea. And we must have an idea to adapt our system. Without the work of Jérôme Salomon [the Director General of Health], we would be in the dark.

How is it that we cannot get more reliable statistics?

In the case of an epidemic, a cohort study must be set up to see clearly. This makes it possible to count everyone, healthy people, like the sick. In the case of the Covid-19, this is going too fast for us to count.

There is an exception to this, it is the Diamond Princess of the name of this boat which remained in quarantine quay in Yokohama with patients on board. It is an experimental model but the Japanese did the study well by testing everyone. In the end, this allowed us to understand that 35 to 50% of patients are asymptomatic. That is to say, they are carriers of the virus but have no symptoms. However, they may be contaminants.

Probably ... But this figure is still much more reliable than the number of people infected. You are the head of emergency at the Georges-Pompidou hospital in Paris. Explain to us how you are organized?

Well, we completely reorganized the service by deploying two very distinct sectors that do not intersect. The first receives coronavirus patients or those likely to be. The second, classic emergencies (heart attack, fractures, etc.), which are not lacking. If you don't do that, the coronavirus will kill more people.

A solidarity movement is being set up around the caregivers. Did you notice it too?

Yes and it feels good. This morning, a baker delivered a whole case of chouquettes to us. The teams appreciated. And then, I receive phone calls from many health professionals who offer their help spontaneously. We are in the process of integrating them into the schedules. For example, surgeons whose non-essential operations have been postponed are giving us a serious helping hand.

Voices are raised for the confinement to be extended beyond two weeks. What do you think ?

To be honest, I hope so! It is clear that the confinement is poorly respected. You have seen, like me, the images of families who continue to walk in the park. It must be understood that confinement saves us time. We have a limited number of reception capacities. And we can't be overwhelmed.

An example ? If 100 serious patients arrive at the Pompidou emergency today, I will not be able to save them all. If these 100 patients arrive within a month, then yes, we can take care of them all. You have to imagine a wave that would be shorter and longer. There is little more than confinement to spread the wave. So people have to stay at home.
