Sichuan doctor Wuhan ICU memory:

There is no longer fear, but it is difficult to fight and disappear and frustration

West China Metropolis Daily-Cover News reporter Yang Shangzhi Wang Xiang Long Tian Xuejiao from Wuhan

My name is Chen Jun. I am a doctor in the ICU of Jianyang People's Hospital of Sichuan Province. On March 5th, it was the 10th day that I entered the ICU ward of Wuchang Hospital of Wuhan City. The patients here were all patients with critically re-crown pneumonia.

I have been an ICU doctor for more than 20 years, and I have seen every part of life and death, but I can't help tearing here many times.

What impressed me most was a couple in their late seventies, who were bedside to bed, unconscious. The daughter left a note on the beam of his father's bed: Dad, you have to be strong, we wait for you to go home. A cell phone is placed on the father's bed, and a daughter's recording is played in a loop: We are at home, and we have prepared your favorite dishes with your mother, just wait for the pot.

But shortly after her mother died, her father failed to survive ...

Support Wuhan

Just got off the plane and the senior made me "prepare for life"

In the Department of Critical Care Medicine at Jianyang People's Hospital, I was a senior doctor. When I walked into the dean's office to receive support tasks, I didn't hesitate.

At 6:30 pm on February 21, after more than two hours of flight, we landed in the empty Wuhan Tianhe Airport.

Going down the ramp, it was gloomy and wondered: Why is the airport so empty and there is only our team on the passage? Then came the reaction-Wuhan closed.

At the airport exit, there were twenty or thirty people who met us. After the car started, I heard many people shouting after the car: Thank you, Sichuan and Wuhan, come on!

When I settled in the hotel, I first called my teachers and friends in the anti-epidemic area of Wuhan. A senior told me: In Wuhan, the ICU ward is the hardest, so get ready to save your life.

We were assigned to the Wuchang Hospital, which is the designated hospital for New Crown Pneumonia.

February 25th was the first day of my "epidemic" battle in Wuhan. Get up at 6:10, get in at 6:50, and arrive at the department at 7:20. Put on your own clothes and pants, wash your hands, wear a mask, hat, and protective clothing ... a complete set of three levels of protection should be fine!

However, the actual situation was not as smooth as I imagined. When I was wearing myopia glasses and wearing goggles, I soon had the most common and most annoying "complications": glasses and goggles immediately fogged!

With the passage of time, the vision blurs and becomes more difficult, and the ears and scalp are getting more and more painful by the lace of the double mask, the sweat under the protective clothing is wet and dry, and the symptoms of hypoxia are getting worse ... During lunch, I almost stumbled from the fifth floor ICU ward to the third floor with the help of two colleagues.

Put on protective clothing again, and don't worry about the heavy fog in the morning, simply don't wear myopia glasses, and then apply the goggles antifog agent carefully, carefully and repeatedly before entering the ward.

It is absolutely invisible in the distance, but it is obviously clear when looking at the near objects! Finally, you can go through the diagnosis and treatment information of each patient more clearly, check the infusion drugs and dosages at the bedside of each patient, and understand the blood purification and ventilator indicators of each patient. The more practical.

Most unbearable

Daughter's recording loop failed to call back his ill father

At 1am on February 27, I went to the night shift and stood on the gate of the hotel on time to wait for duty. The night light in the drizzle seemed extra lonely.

Wuchang Hospital, which is shrouded in darkness, looks peaceful. The lights at the entrance of the emergency building reflected the red lanterns hanging high before the Spring Festival, which seemed a bit inappropriate, and made the "isolation lines" under the red lanterns more dazzling.

At this time, the ICU ward was still busy, and the ventilator and monitor issued various harsh alarm sounds, one after another, which set off the night's silence.

A couple in their late seventies, about 10 days ago, were admitted to the ICU one after the other due to new coronary pneumonia, and their conditions were almost the same: extreme hypoxemia, which requires strong support from a ventilator; severe hypotension, two types of boost The combination of drugs and large-dose maintenance; serious disturbance of renal function and internal environment requires continuous blood purification. The two joint rounds guidance by national and western medical experts have had little effect.

Before we went to work that day, the old lady had already left, leaving her husband on the bed still struggling to support him. At this time, he could not perceive anything.

A note was affixed to the beam above the head of the old man, written by his children: "Dad, you must be strong, we wait for you to go home, it must be ..." Unfortunately, we cannot bring a cell phone into the ICU, and we cannot take this picture. Moving note. There was a mobile phone next to the old man's pillow, and his daughter's message was played in a loop: "Dad, when are you going home? We have already prepared your favorite dishes with your mother, just wait for the pot ...".

In such a time and space, people's minds are uncomfortable and indescribable, as if it is an endless long night ... until 8 am to change shifts.

Four days later, the old man who had been unconscious also left. I still don't know which doctor or nurse helped him to put his mobile phone on the bedside and repeatedly played the voice from his daughter.

I wanted to ask a colleague several times, but I couldn't open the mouth.

Best wishes

If the patients are doing aerobics like the square cabin hospital ...

On the day I first arrived, there were 12 patients in the ICU, 6 of whom were intubated. By March 2, these 6 patients had left.

Over time, the inner tension and fear diminish, but the loss and frustration are hard to dispel, and there is anxiety.

For example, a mother-in-law in bed nine suffered a cardiac arrest. Because two boosters were pumped at the same time, the blood pressure was barely able to be maintained, and it had been under the strong support of the ventilator for more than two days. More unexpectedly, after about 5 minutes of routine rescue, her heart beat actually recovered, and the indicators did not worsen significantly. But the next day, I went to bed 9 again, only to find that the bed was empty and the mother-in-law was no longer there. The original "Ang Li" is just a temporary "illusion". Ugh……

Wuchang Hospital's ICU receives all (critical) critically ill patients. We have admitted 18 patients one after another. The condition is still a long way from being discharged. The patient's condition has improved a little and is stable, all like a ray of sunlight shining into our hearts.

On March 2, we received a 47-year-old patient who was in a very bad condition at first. He could only continue to inhale high oxygen while lying on the bed, and even struggled to turn over.

There is no specific medicine for New Coronary Pneumonia. In addition to conventional oxygen therapy, symptomatic medication, and efforts to improve lung function, all we can do is to find ways to enhance patients' immunity and self-healing ability.

When exchanging conditions, I comforted him and said, don't lie in bed all the time, try to move, even if you sit in bed for a while. If you can't do it now, you can test whether you can lie on your side or prone while sleeping. Later, try sitting next to the bed, even walking around the bed, step by step.

A few days later, with our joint efforts, the patient was able to sit on the bed and eat porridge with a spoon, although he could not breathe after a few bites. For doctors, this change is pleasing.

Recently, the news reported that some patients can do aerobics in the cabin hospital. If the patients in our department can jump up aerobics as in the Fangcai Hospital, then my eyes will really smile.

Behind the ICU

Colleagues joked "have a relaxing Spring Festival"

When I am busy, I will chat with colleagues. A doctor from Wuchang Hospital talked calmly about his most likely infection experience: wearing a surgical mask frequently to consult patients diagnosed with severe new coronary pneumonia; emergency tracheal intubation for patients without special third-level protection measures Tube, a lot of bloody sputum sprayed on the work clothes.

Moreover, it was confirmed from his mouth that since the beginning of January, the ICU ward has been fully occupied with 14 critical numbers, and there are only 5 doctors on duty in the department, who rotate day and night, and most doctors have never returned home since then.

Director Xu of ICU joked that this year's Spring Festival is very easy and simple, that is, from the department to the hotel at two and one line, so as not to get a red envelope from the New Year, it really has a "clean" Spring Festival.

The work of the ICU is not simple, especially when wearing protective clothing for a long time can cause all kinds of discomfort. Every day after work, I went back to the hotel to take a shower, leaned on a chair, and put my feet on the bed. I was so tired that I didn't want to move.

They also call home during breaks.

My wife and I are traveling together. She can understand that what cares most is how to eat and rest in Wuhan. Mom and Dad occasionally call just to say whether they have eaten or not, and work is rarely mentioned.

I still remember the scene of getting off work on the first day. After 8pm, Wuhan was raining lightly and walked to the window. The neon lights in the distance were faintly visible, although blurred.
