A group formed by bereaved families calls for a review of the medical accident investigation system. The system treats the unexpected death of a patient as a "medical accident" and requires all medical institutions to report to a third party and investigate, but the decision is left to each medical institution.

Family members have also pointed out that the system is not functioning properly due to the lack of support. The number of reported accidents per population varies by about five times depending on prefecture, according to the Japan Medical Safety Research Organization.
