Once the patient returned home for the second time, she had a heart attack.

She was then taken urgently to the same clinic where doctors after X-rays could establish that there were several blood clots.

During the two previous visits, the patient must have been told that it was not a plug as the leg was not "red or swollen", according to the report.

The complainant writes that the doctors should have understood that the risk of blood clots was high considering that she had cancer and underwent chemotherapy.

The patient died shortly thereafter.
