On June 17 this year, 44-year-old Mikael is found dead in his apartment.

Despite housing support Monday to Thursday every afternoon, he could lie dead for days without being detected.

Mikael's friends and relatives have since tried to get answers about what happened and when the municipality last visited him.  

SVT has in a number of articles told about the case of Mikael and now the municipality's own investigation is complete - an investigation that is done six months after Mikael's death and only after SVT has been able to show several inaccuracies about the municipality's actions. 

The first investigation - a "post-construction" 

The investigation shows that the head of unit who investigated the incident and who found that there were no irregularities in fact created the case three months after Mikael was found dead and that the authenticity of the first investigation is impossible to prove: 

"This is a post-construction to protect the reputation of the business," the investigation states.

No one knows when they last visited Mikael

The investigator also states that the municipality does not know when they were last home to Mikael - despite the fact that he was granted support four days a week. 

They were probably at Mikael's home a week before he was found dead - but neither is it recorded due to "substandard documentation".

In the investigation, the municipality also claims that they did not visit Mikael when he did not answer and because they did not have a key, something that relatives say the municipality should have had access to. 

Overall, the investigator states that the incident is to be regarded as a serious misconduct according to lex Sarah and should have been reported as such immediately.

Gällivare municipality now reports themselves to the Swedish Health and Care Inspectorate.
