Have a baby hurt?

mistaken!

Reducing medical intervention in obstetrics with non-invasive and painless technology becomes a trend

China Maternal and Child Health Association will hold the 16th "International Obstetrics and Pediatric Management Service New Model Demonstration Project Promotion-and Exchange on Clinical Application of Obstetric Non-invasive Technology and Painless Delivery" at Beijing United Family Hospital from October 15 to 17, 2021 .

It aims to learn international advanced obstetrics and pediatrics service concepts, popularize non-invasive obstetrics technology, reduce medical interventions, change concepts, reduce birth trauma, and improve women and children's health.

The traditional concept of "giving a baby must hurt" is being subverted.

Clinically, modern people are more accepting of new technologies and have higher requirements for quality of life, especially those born in the 80s and 90s who are in the reproductive age, are more willing to accept non-invasive and painless delivery techniques.

  Non-invasive midwifery painless delivery

Has become a consensus in the medical field

"Any medical behavior should minimize the interference to the patient's body, from large incisions to small incisions, from'minimally invasive' to'non-invasive', so that patients can obtain the best medical results at the least cost, and the concept of non-invasive and painless It has become a lifelong goal pursued by doctors.” Yu Xiaoqian, executive vice president and secretary-general of the China Maternal and Child Health Association, said that the association has promoted non-invasive midwifery and painless delivery for many years. From a purely technical point of view, it promotes resistance. It is mainly a conflict between traditional technology and ideas.

"Many people think that childbirth is painful and happy, and childbirth is painful." But Yu Xiaoqian said that now he emphasizes humanistic care, hoping to relieve the "sickness" and the "pain" at the same time.

The concept of non-invasiveness and painlessness has been highly accepted by the medical industry, and everyone has reached a consensus that more care should be given to the pregnant women.

At present, the country’s overall fertility rate is declining, but Yu Xiaoqian believes that the liberalization of the three-child policy will bring development opportunities to the industry. “As long as it reaches the bottom, it will surely rise.”

The maternal and child industry can seize the high-quality development during the transition period, and improve in all aspects from the perspective of service system, technical system, personnel quality, and humanistic care.

Taking United Family as an example, it has now been transformed into a general hospital. However, in the early days of its establishment, the hospital started from women and children and was constructed in accordance with international standards and medical industry norms. The hospital has always advocated the concept of non-invasive midwifery. Low lateral incision, non-invasive forceps, VBAC trial production, painless delivery and other technologies have led the concept of reducing childbirth trauma and are at the forefront of the country.

Reduce cesarean section and reduce lateral cut

Non-invasive midwifery guarantees the health of pregnant women

According to Yu Shasha, director of the obstetrics department of Beijing United Family Hospital, for many women, childbirth causes damage to multiple organs of the body. What obstetricians need to do is to minimize or avoid these injuries. This is also the purpose of non-invasive delivery.

"In recent years, the obstetrics community has been advocating'reducing unnecessary interventions and returning to nature as much as possible', which refers to reducing the cesarean section rate and giving birth as smoothly as possible." Yu Shasha said, especially when the three-child policy was liberalized. Under the background, it is necessary to reduce the harm to the mother and make better preparations for the future delivery.

If a woman feels safe and comfortable psychologically and physically when she gives birth to her first child, she will be willing to have a second or third child.

Yu Shasha said that during vaginal delivery, in many cases, lateral perineal incision can be avoided, but because of the shortage of medical resources, many obstetricians will choose lateral incision to increase the speed of delivery, and the wound is flat and easy to suture.

For this reason, the lateral cutting rate of some medical institutions is even as high as 80% to 90%.

"But if conditions allow the obstetrician to wait patiently and allow the mother's vagina to expand better, he can reduce or avoid lateral cuts and minimize the harm to the body." At present, the rate of lateral cuts at United Family is less than 20%, and now public hospitals It is also advocating to reduce this ratio.

Yu Shasha introduced that since the liberalization of the second-child policy, the proportion of high-risk women over 35 years old in the hospital has accounted for more than 60%, and it is still increasing year by year.

VBAC (Vaginal Subsequent Delivery after Cesarean Section) technology enables women who have undergone a first-born cesarean section to give birth as much as possible during their second and third births, thereby reducing physical damage.

VBAC technology puts forward extremely high requirements on the overall capabilities of the hospital. Successful implementation requires the hospital to provide safety protection measures. In addition to the "purple code" of the obstetric emergency response process, anesthesiologists, pediatricians and operating rooms can provide 24 hours a day service. In the event of a problem, medical staff have the ability to respond to emergencies and immediately perform cesarean section operations to maximize the safety of the mother.

At present, the success rate of VBAC in this hospital is above 80%.

Relations to women's and children's health

  Painless childbirth promotion is imperative

Recently, a 30-year-old pregnant woman gave birth to her first baby in United Family.

She joked to the medical staff and said: "You are doing a job for my husband." It turned out that she had resolutely told her family before that she was determined not to give birth after giving birth.

But after experiencing a painless childbirth, she felt that childbirth was not as painful as she had previously expected, so she took the initiative to ask her family: "Why don't you fight for three years?"

Pan Zhongying, director of Beijing United Family Hospital, introduced that painless childbirth in China is a process of "make up lessons".

The hospital has adopted painless delivery since its establishment in 1997 for the first baby delivered. It is also the first to introduce advanced international delivery concepts into domestic medical institutions.

After 20 years of development, this continuously improved system has been standardized and promoted by the country's comprehensive promotion of painless childbirth.

"We will not put these things down. The health of women and children is related to thousands of households. Sharing experience can reduce women's injuries caused by childbirth, which is of great significance in itself."

According to Liu Wei, the medical director of the Beijing North Women’s and Children’s Hospital, United Family is the first batch of national labor analgesia pilot hospitals. He also participated in the preparation of technical guidelines and management guidelines for childbirth promoted by the National Health Commission and integrated into the hospital. Fang has accumulated many years of experience.

In addition to painless childbirth, pregnant women at United Family can also receive a full range of chain services from prenatal, during and after delivery.

Pregnancy management is led by the obstetrics department, with the cooperation of midwife clinics, anesthesiologists clinics, nutrition clinics and multidisciplinary clinics.

Obstetricians are equivalent to general practitioners, and they can promptly determine whether the problems encountered by pregnant women need to be solved by other departments, including the treatment of complications such as hypertension and diabetes, as well as nutritional conditioning, mental health and other issues.

"Providing a most suitable integrated solution for pregnant women is an area that the hospital will further deepen and expand in the future." Liu Wei said.
