Go deep into the grass-roots site to strictly supervise and carefully inspect, expose problems, identify outstanding problems and shortcomings that affect current safety production work, and prevent and contain major production safety accidents.
Recently, The Paper accompanied the First Working Group of the Office of the State Council’s Security Committee to Investigate Unannounced Visits (hereinafter referred to as the "Unannounced Visits Group") to Jinzhong City, Shanxi Province, and inspected six of the three districts and counties of the city, including Shouyang, Yuci, and Zuoquan. For coal mines, focus on inspection and analysis of 2 normal production mines.
"From the inspection situation, there are still many problems and hidden dangers, and some problems and hidden dangers are more serious." Yang Yimin, the leader of the unannounced visit team and deputy director of the accident investigation and statistics department of the National Mine Safety Supervision Bureau, told The Paper. There are widespread problems in mine inspections, such as inadequate implementation of the three-year special rectification of coal mine safety production, inadequate advance control of gas and other major disasters, and insufficient safety supervision.
Gong Qi, member of the Standing Committee of the Jinzhong Municipal Party Committee and Executive Deputy Mayor, stated that the unannounced visits team will "accept all the problems and make serious rectifications" and will systematically sort them out and implement them one by one.
"Make great efforts to solve our existing problems, and some mines really should stop production for rectification."
Disaster prevention is not in place, and gas prevention and control should be paid attention to
Gas prevention and control has always been the focus of the coal industry.
Gas accidents occur frequently, causing loss of personnel and property. There is a saying in the industry that “gas is dead, and mines will have no peace”.
Since the beginning of this year, two coal mine fatalities have occurred in Jinzhong City, one of which was a major gas outburst.
The unannounced visit team's inspection found that some coal mines have many problems in the advance control of major disasters, and the prevention and control of gas is not effective.
In the Jingfu Coal Mine in Shouyang, when The Paper followed the unannounced visit team to go down to check, from time to time, the sound of the coal body in the coal mining face was heard.
In this regard, the experts of the unannounced visit group explained that this is a "coal cannon" sound, which is caused by the sudden release of coal seam gas pressure or ground stress, and is a manifestation of coal seam dynamics.
In the face of frequent occurrence of "coal artillery", Jingfu Coal Mine failed to complete the identification of coal and gas outburst hazards as required.
Experts told The Paper that without the appraisal, it is impossible to accurately grasp the hazards of coal and gas outbursts in the coal mine. Only through the appraisal can relevant control measures be taken in a targeted manner. Now it is basically blind governance."
At the same time, the unannounced visit team found that there were also problems with gas drainage in Jingfu Coal Mine.
All the single-hole control gate valves of the gas drainage pipeline in the 15107 coal mining face of the coal mine failed, and the drainage pipeline could not discharge water normally, resulting in poor drainage effect.
Experts further introduced to The Paper that the gas drainage effect is poor, and the gas in the coal body cannot be discharged. It is necessary to reduce the gas concentration in the underground air flow by increasing the ventilation and increasing the wind speed, but this will cause new problems. .
"If the wind is strong, the coal dust in the mine will be large." This will not only affect the health of the underground workers, but also cause the safety hazards of spontaneous coal seams.
In addition, the experts of the unannounced visit team inspected the coal mine safety monitoring system and found that the coal mine professionals did not take corresponding measures when abnormal conditions such as the interruption of the gas monitoring and early warning curve occurred in the system.
Similarly, there are many flaws in the gas prevention and control work of the Beishan Coal Mine in Yuci.
When the unannounced visit team inspected the No. 8 coal seam operation area of the mine, it was found that the gas pressure and gas content were not measured at the correct location.
The unannounced visit team also found that the gas content of the No. 8 coal seam had reached 11m/min, but the first extraction and then excavation were not carried out.
At the same time, the mine has not formulated management systems such as tandem ventilation gas daily analysis, hot operation, etc., and has not prepared an emergency plan for the shutdown of the main ventilators.
In addition to gas disasters, water prevention and control measures in the two production mines are also inadequately implemented.
In the 15107 working face of Jingfu Coal Mine, the coal mine did not conduct drilling verification when the abnormality in the front area was detected by professional instruments.
Beishan Coal does not have the minutes of monthly water prevention and control office meetings.
Safety basic management is chaotic, ten instruments and nine are broken
In the two production mines inspected by the unannounced visit team, there were different levels of safety management problems.
For example, coal mines have inaccurate mastery of regulations and standards.
The unannounced visit team found that the annual ventilation capacity of the Jingfu and Beishan mines had not been approved according to the annual excavation plan.
Experts said that according to relevant regulations, coal mines must conduct annual ventilation and production capacity verification. "The ventilation and production capacity of coal mines must be matched. They must be'determined by wind', and how much work they can do."
The two coal mines also have insufficient technical management.
For example, the design of No. 15 and No. 6 coal mining areas of Jingfu Coal Mine, and the setting of coal pillars at the coal mining face have no basis.
Unannounced interview group experts told The Paper that these should be calculated and designed based on the structure of the coal seam, the lithology of the coal seam roof, and the appearance of pressure.
During the actual inspection of the project, the unannounced visit team found that there were problems with the on-site management of the two coal mines.
In the 15107 coal mining face of Jingfu Coal Mine, 9 of the 10 pressure gauges used to observe the supporting force of the hydraulic support were damaged.
According to experts, the pressure gauge can show the working status of the hydraulic support. Once an abnormality occurs, the hydraulic pressure should be added immediately or repaired and replaced according to the situation.
In addition, the metal baffle of No. 1206 feed switch bell mouth of the second belt head power distribution room in Jingfu Coal Mine is corroded, and the starter switch load line No. 2605 of the winch power distribution room of Beishan Coal Industry Co., Ltd. in the east mining area is loose. It belongs to the phenomenon of missing explosion. In addition, the safety monitoring system of Beishan Coal Industry Co., Ltd. and the personnel position monitoring system are not synchronized with the underground data, and the system functions are not perfect.
"These are all hidden dangers of major accidents." The unannounced visit group expert told The Paper.
In addition, the roof of the 15107 mining face of Jingfu Coal Mine is severely separated from the layer, which has been in a "U" shape.
Experts told The Paper that this is a roof management problem. At the scene, the experts pointed out to the coal mine managers who accompanied the coal mine for inspection very strictly: "The roof support is improperly supported and cannot be produced!"
The safety management of Beishan Coal Industry has also been criticized by the unannounced visit group. The leader Yang Yimin said: “On-site management can be said to be a mess.” The unannounced visit group found that the mining period of the coal mine in this mine is less than 2 months, which is much lower than that of the mine. The standard of less than 4 months.
"It is impossible to guarantee the balance of the mining period. The excavation progress and coal output fluctuates from high to low, which is prone to super-power and super-intensity production." The members of the unannounced visit group introduced the paper to The Paper.
In addition, the mine's management personnel are insufficient and the level of quality is not high.
When The Paper followed the unannounced visit team to inspect the mine’s safety monitoring system, the staff on duty had difficulty answering the experts’ questions.
Experts from the unannounced visit group told The Paper that through inspections, it was found that the staff on duty were not familiar with the operation of the system at all, nor did they take timely measures and record the long-standing system alarms, failures, and data interruptions.
During the inspection, the experts also found that the personnel information statistics function in the personnel location monitoring system of the mine was faulty, and the personnel information statistics data were lost at a fixed time every day.
"Once an accident occurs, this will have a great impact on accident rescue."
Relevant departments responded quickly and rectified the shutdown of mines with serious problems
The Paper learned that during this inspection, the unannounced visit team found nearly 70 problems in the two production mines.
Gong Qi, the executive deputy mayor of Jinzhong City, said that for the unannounced visits team’s “accept all the issues, and earnestly rectify them”, they will systematically sort out the problems and implement them one by one.
On August 3, The Paper learned that local rectification measures have been taken.
(1) In response to the problems existing in Beishan Coal Mine, the People's Government of Yuci District has adopted the following measures:
First, on the morning of August 31, the district government organized the actual controllers and mine directors of all coal mines to hold a special meeting. The actual controllers of Beishan Coal Mine conducted inspections and talked about measures. Other mines learned from one another and learned lessons.
Second, Beishan Coal Mine immediately stopped production for rectification, formulated a rectification plan, clarified the measures, time, responsible person, and funds, and reported to the district for emergency review after the main enterprise audit, and started rectification after meeting the standards. If the rectification is not in place, production will not be resumed.
Third, the District Emergency Management Bureau immediately issued an order to stop production, and the District Emergency Management Bureau and Wujin Coal Industry Company respectively arranged special personnel to stay in the mine to guard against the actual production and construction in the name of rectification; at the same time, they gave advice on how to deal with the supervision personnel of the package mine.
Fourth, other coal mines in the district immediately carried out self-examination and self-improvement against the existing problems in Beishan Coal Mine, formalism resolutely suspended production for rectification, notified the district, and re-executed the procedures for resuming production and work.
(2) In response to the problem of high roof pressure and layer separation in the belt along the leading support section of the 15107 fully-mechanized mining face of Jingfu Coal Mine, the People's Government of Shouyang County has taken the following measures:
First, on the evening of July 30th, the Shouyang County People’s Government organized the county’s coal mine chairman, general manager, safety chief, and chief engineer to convene the July coal mine safety regular meeting and the National Mine Supervision Bureau’s inspection issue briefing, requesting all the county’s mines to learn from each other. , Learn from the lessons, and carry out self-examination and self-correction work in response to problems.
The second is that Jingfu Coal Mine ceased the production activities of 15107 working face on July 31 under the condition of adopting reinforcement and support measures.
Third, on August 1, the County Emergency Management Bureau, Huayang Group, and Jingfu Coal Mine hired experts to conduct a consultation on the 15107 working face of Jingfu Coal Mine and formulate a realistic remediation plan.
Completely eliminate hidden dangers, and do not organize production before the hidden dangers are rectified. After the rectification is completed, a special report shall be made at the executive meeting of the government.
The unannounced visit team asked the local area to conduct a review before the end of August, "look back"
The unannounced visit team also made targeted suggestions for the problems investigated.
One is to keep an eye on gas governance.
Supervise all coal mining enterprises to strictly implement the relevant notification requirements, strengthen the daily operation and maintenance of the safety monitoring system, and establish a complete monitoring and alarm emergency response system to ensure the normal operation of the system.
Strictly enforced inspections of gas over-limits and found that coal-mine gas over-limits must be investigated and dealt with in accordance with the requirements of the “Measures for the Investigation and Handling of Major Hidden Hazards”, and severely held accountable.
Resolutely prevent coal and gas outburst accidents.
At the same time, the prevention and control of water hazards cannot be ignored.
The unannounced visit team pointed out that Shanxi has many coal mines with integrated resources, abandoned small kilns and old goafs are all over the minefields, and the hidden disaster-causing factors are unclear.
Under this situation, the unannounced visit team suggested that all those who fail to be equipped with geological prevention and water professional technology as the vice president of geology should be suspended for rectification; for hidden disaster-causing factors, water damage control is not in place, and the effectiveness test fails to meet the standards, all production should be suspended for rectification. ; Those who do not stop production and withdraw their personnel in extreme weather shall be held accountable seriously.
Strictly prevent accidents such as water penetration and flooding.
In addition, the unannounced visit team put forward suggestions for the in-depth promotion of the three-year action and large-scale investigation of the special rectification of coal mine safety production.
In the course of this inspection, it was discovered that the local government did not implement the three-year special rectification actions in place.
For example, the Shouyang County Local Coal Mine Safety Supervision Administration did not formulate a coal mine safety production inspection implementation plan for the county's coal mine safety production work, but merely forwarded documents from higher-level departments.
In this regard, the unannounced visit team asked the local government to review the three-year action and large-scale investigation work before the end of August. "Four 100%" such as supervision and law enforcement.
If the large-scale investigation is not carried out seriously, the risk research is not in place, the rectification of hidden dangers is not thorough, the problem is not accurately identified, and the measures are not specific, they shall be ordered to overthrow and start over and make up classes again.
At the same time, the unannounced visit team specifically pointed out that it is necessary to strictly investigate and deal with violations of the law and regulations such as ultra-stratification and cross-border mining.
According to reports, since the beginning of this year, under the condition of continuous high prices of mineral products, some coal mine enterprises have caused outstanding problems such as mining misalignment, inadequate management of major disasters, rush to schedule, and rush to progress, which are likely to cause more than larger accidents. .
Government departments and mining enterprises at all levels must severely crack down on illegal production activities of mining enterprises with super-capacity, super-strength and over-fixed personnel, and it is strictly forbidden to mine beyond the boundary.
