The social services in Tranås had knowledge that the man had used violence against the woman on several occasions and the woman was placed in sheltered housing in 2015 due to the abuse.

But despite this, the investigation shows that there have been opportunities for the suspect to commit lethal violence.

The safety of the children was taken into account in all situations, but the right measures were not taken for the woman.

- We have a great responsibility.

We have not contributed to the man murdering the woman, but we have failed in the risk assessment.

We have not really thought about the consequences of the violence, says Angelica Freeman, section manager of the social services in Tranås.

Better risk assessments

The man and the woman had children together who were cared for by family homes in both Tranås municipality and Motala municipality.

Both of them had contact with the children where staff from the municipalities were involved.

At the time of the murder on April 15, Tranås municipality had just withdrawn from the visit, but was thus involved in the previous visit.

- The parents met the children together.

With the knowledge we had about the woman's situation, we should have questioned it, says Angelica Freeman.

Could this tragic event have been avoided?

- We do not know.

But we should make better risk assessments to minimize that this could happen.

Registration to IVO

The independent review of the municipality's actions has been carried out by specialized investigators through a conclusion company.

The result of the investigation has led the municipality to choose to make a report to the Swedish Health and Care Inspectorate according to lex Sarah.

- There are serious shortcomings and we feel a great responsibility.

As a social service, we must never let go of the guard.

The municipality of Tranås has also taken extensive measures to increase competence linked to the work against violence in close relationships.

The municipality of Motala has also made a lex Sarah report regarding its actions around the incident.
