The characteristics and common problems of these accidents are obvious, fully revealing that some companies have insufficient and inadequate knowledge of the risk identification of limited space operations; failure to implement the limited space operation approval system and operating procedures; contracting limited space operations to those who do not have safe production conditions The unit is managed in a package, and one package is provided; the necessary protective equipment and emergency equipment are not provided in accordance with the regulations; safety education and training are not in place, the operators habitually violate the rules and regulations, and the blind rescue in emergencies leads to prominent problems such as increased casualties .

  The Emergency Management Department emphasized that limited space operations involve mining, chemical, construction, electric power, papermaking, shipbuilding, food processing, catering, municipal engineering, urban gas, sewage treatment, special equipment and other industries, which are high risks that companies can easily ignore. Operation, in addition to the vastly different operating environment and uneven quality of the operating personnel, it is extremely prone to production safety accidents in limited space operations.

All enterprises must strictly implement the main responsibility of safe production, and effectively prevent and control risks in limited space operations.

Determine the basic information of the limited amount of space, location, and hazardous and harmful factors in the enterprise, and establish a limited space management ledger.

1. The "2·15" major poisoning accident of Shuangzhou Paper Co., Ltd. in Zhongtang Town, Dongguan City, Guangdong Province

  On February 15, 2019, the director of the Environmental Protection Department of Dongguan Shuangzhou Paper Co., Ltd. in Guangdong Province arranged 2 workshop directors to organize 7 workers to clean up the sewage conditioning tank (accident emergency tank).

At about 23:00 that night, three operators inhaled hydrogen sulfide in the pool and fainted after being poisoned. The personnel outside the pool immediately called for help. Six people went to the pool for rescue. Five of them fainted in the pool and one felt it. Climb out without discomfort.

The accident eventually caused 7 deaths and 2 injuries, and the direct economic loss was about 12 million yuan.

The main lessons: First, the company failed to perform the approval procedures for limited space operations, did not conduct inspections, did not ventilate before the operation, and the operators did not wear personal protective equipment, and entered the limited space for operations in violation of regulations.

Second, after the accident, on-site personnel blindly rescued, causing increased casualties.

The third is the lack of corporate emergency drills, untrained operators, and lack of limited space safe operation and emergency response capabilities.

Accountability: Six persons including the legal representative of Shuangzhou Paper Company, the person in charge of the production department, the manager of the personnel administration department, the safety management personnel, the director of the environmental protection department and the sewage treatment team leader were transferred to the judicial authority for processing, and the company was subject to administrative penalties .

2. "September 23" major poisoning and suffocation accident on Zhengdian Street, Jiangxia District, Wuhan City, Hubei

  Wuhan Jiangxia Road and Bridge Engineering Corporation is responsible for the dredging operation of the underground sewage pipe network of Fengyang Avenue, Zhengdian Street, Jiangxia District, and entrusts the dredging operation to Wuhan Degan Road & Bridge Co., Ltd.

At around 11:30 on September 23, 2019, Wuhan Degan Road & Bridge Co., Ltd. conducted a dredging operation in a sewage inspection well on Fengyang Avenue. One on-site personnel fainted while entering the well. The other three on-site did not take any action after finding out. Protective measures went down the well to rescue people, and accidents of poisoning and suffocation occurred, which eventually caused 3 deaths and 1 injury, and a direct economic loss of about 3.9106 million yuan.

The main lessons: First, Wuhan Jiangxia Road and Bridge Engineering Corporation, as the contracting unit, did not sign a safety production management agreement with the subcontractor, and allowed the subcontractor to perform illegal operations.

Second, as a labor subcontractor, Wuhan Deccan Road and Bridge Co., Ltd. does not fulfill its responsibility for production safety and does not have the conditions for safe production to engage in related operations. Temporary recruiters organize personnel to carry out limited space operations without safety education and training; Take any safety protection measures, and after the accident, the blind rescue has led to increased casualties.

Responsibility: Two persons including the legal representative of Wuhan Deccan Road and Bridge Co., Ltd. and the person in charge of the project site of Wuhan Jiangxia Road and Bridge Engineering Corporation were transferred to the judicial organs for handling; the other 13 persons were given party discipline and government sanctions; administrative penalties were imposed on two units responsible for the accident .

3. "12·31" major poisoning accident of Xuzhou Tianan Chemical Co., Ltd.

  At 20 o'clock on December 31, 2019, the personnel of Chongqing Huawei Air Liquide Equipment Manufacturing Co., Ltd., a contractor of Jiangsu Xuzhou Tianan Chemical Co., Ltd., blindly discharged desulfurization liquid during maintenance operations in the desulfurization tower, causing the liquid seal to fail, and the pressure was held in the circulation tank. The gas in the upper space broke through the liquid seal and entered the tower, resulting in a poisoning accident for five construction workers in the tower, of which three died after rescue efforts failed. The direct economic loss was about 4.02 million yuan.

The main lessons: First, Tianan Chemical lacked safety management, unable to provide relevant information about the change of the desulfurization tower, failed to review and found that the contractor did not have relevant qualifications, and failed to apply for restricted space operation tickets.

The second is that Chongqing Huawei illegally undertook it, did not provide safety training for its temporary employees, did not apply for restricted space operations, and failed to rescue when the accident occurred.

Responsibility: The legal representative and general manager of Tianan Chemical Industry and the legal representative of Chongqing Huawei will be investigated for criminal responsibility.

4. "5.1" major accident in the sewage pipe network repair project in Fanchang Economic Development Zone, Wuhu City, Anhui Province

  Shanghai Qianye Municipal Engineering Co., Ltd. arranged part of the auxiliary works in the sewage pipe network restoration project of Fanchang Economic Development Zone to Huangshan Branch for construction, and Huangshan Branch also verbally arranged to Ningbo Boyu Environmental Engineering Co., Ltd. for construction.

At around 11 o'clock on May 1, 2020, Ningbo Boyu Environmental Engineering Co., Ltd. was in the process of maintenance and construction of the second phase of the non-excavation repair of the sewage pipe network in Fanchang Economic Development Zone. Due to the improper position of the water gun nozzle, it was necessary to go down and adjust, and one construction worker The personnel only wore waterproof clothing and safety helmets to go down the well and then fainted.

Two other people at the scene went down to rescue and fainted after being discovered. A poisoning and suffocation accident occurred, which eventually caused three deaths and a direct economic loss of 4 million yuan.

Main lessons: First, Shanghai Qianye Municipal Engineering Co., Ltd., as the project contracting unit, lacked the safety management of the contracting project, and failed to detect, stop and correct the illegal operations of on-site construction personnel in a timely manner.

The second is that Ningbo Boyu Environmental Engineering Co., Ltd., as a subcontractor, organizes workers who do not have the basic knowledge of working safety in limited space to carry out the maintenance and construction of the sewage pipe network, and does not provide the workers with the necessary labor protection equipment.

Responsibility: Administrative punishment was imposed on 6 persons including the legal representative of Ningbo Boyu Environmental Engineering Co., Ltd.; administrative punishment was imposed on 2 units including Ningbo Boyu Environmental Engineering Co., Ltd.

5. Henan Jiaozuo Minnong Noodle Products Co., Ltd. "July 18" major poisoning and suffocation accident

  The Zhangcaiyuan Village Committee of Zhandian Town, Wuzhi County, Jiaozuo City, Henan Province leased non-construction land to Ruidu Flour Processing Factory in Wuzhi County for food production, and Ruidu Flour Processing Factory in Wuzhi County transferred part of the site to Jiaozuo Minnong Company is engaged in food production.

At about 18:00 on July 18, 2020, a worker of Jiaozuo Minnong Company fell into the material tank due to improper operation while working in the fermentation workshop, resulting in poisoning and suffocation. Other workers handled improperly and blindly rescued, resulting in the consequences of the accident. The expansion caused a total of 6 deaths.

The main lessons: First, Jiaozuo Minnong Company violated the law and regulations for production and operation without the food production license, and refused to implement the relevant department's order to stop production and conduct production without authorization.

Second, the limited space management system of Jiaozuo Minnong Co., Ltd. is lacking, risk identification and hidden danger investigation and management are not organized, and on-site management is chaotic.

The third is that the safety training of Jiaozuo Minnong Company is not in place, the safety awareness of employees is weak, and the emergency response ability is poor.

Fourth, Zhangcaiyuan Village Committee in Zhandian Town, Wuzhi County, and Ruidu Flour Processing Plant in Wuzhi County illegally leased non-construction land to Jiaozuo Minnong Company, which does not have the conditions for safe production.

Responsibility: The legal representative of Jiaozuo Minnong Co., Ltd. died in the accident and was exempted from criminal responsibility. The legal representative of Wuzhi Ruidu Flour Processing Factory, the party branch secretary of Zhangcaiyuan Village, Zhandian Town, Wuzhi County, etc. Two persons were investigated for criminal responsibility; the business licenses of Jiaozuo Minnong Company and Wuzhi Ruidu Processing Factory were cancelled, and they were closed down according to law.

6. "10·30" accident of Shaanxi Jingyi Chemical Co., Ltd. in Shenmu City, Shaanxi Province

  At about 17:00 on October 30, 2020, during the trial production and commissioning of Shaanxi Jingyi Chemical Co., Ltd. in Shenmu City, Yulin, Shaanxi Province, a nitrogen suffocation accident occurred in the sewage treatment tank of the coal tar pretreatment unit, causing 3 deaths and 1 injury.

The reason for the preliminary analysis is that one employee on duty suffocated when he entered the tank from a manhole illegally without performing gas detection and analysis on the tank, without obtaining permission to enter the restricted space, and without taking personal protective measures, and the other 2 people were suffocated when they entered the tank from a manhole for inspection. An accident occurred while wearing a long-tube respirator and one person wearing an air respirator entered the tank for rescue, resulting in increased casualties.

The main lessons: First, there are loopholes in the implementation of the company's safety production system, false safety education and training, low safety awareness of grassroots employees, insufficient awareness of potential risks, and lack of knowledge and skills.

The second is the failure to properly wear and use personal protective equipment and rescue equipment when carrying out emergency rescue.
