The child would have had surgery for his heart defect at the Children's Heart Center at Drottning Silvia's Children's and Adolescents' Hospital at Sahlgrenska University Hospital in 2019. But other operations preceded and the child was postponed again and again, something that GP was the first to report on.

When the operation was moved forward for the seventh time, the child and his family were at home.

Then the child suffered a cardiac arrest and died.

Criticized by Ivo

In a decision, the Swedish Health and Care Inspectorate (Ivo) writes that the hospital broke down when it was to "plan, lead and control the activities in a way that leads to the requirement for good care being maintained".

The hospital is also criticized for shortcomings in record keeping.

According to the hospital's internal investigation, daily checks were made on the child, but these were only documented if anything deviated from normal.
