How did the error occur?

The National Eye Center of Singapore said that the agent responsible for diluting the vaccine was called to another order before diluting the vaccine, then another employee made a mistake, not knowing that the dose in the vial was not diluted, and gave the vaccination to the health worker concerned.

The bug was discovered within minutes after vaccination.

According to the US Centers for Disease Control and Prevention, the preparation of the Pfizer-Biontech vaccine includes diluting the vaccine ampoule with an amount of 1.8 milliliters of saline solution, then 0.3 milliliter of it is withdrawn, and injected to the person to be vaccinated.

Therefore, the mistake that was made, which was not diluting the ampoule, led to the health worker receiving 5 doses of the vaccine at once.

Professor Wong Tian Yin, Medical Director of Singapore's National Eye Center, said the center apologized to the injured employee and his family, conducted a comprehensive review of its internal processes, and took steps to tighten them so that such lapses do not recur again.

For its part, the Ministry of Health said that it had worked with the Singapore National Eye Center to identify the vulnerabilities that contributed to the error.

The ministry said clinical trial data from Pfizer-Biontech indicated that receiving more than the recommended dose of the vaccine was unlikely to be harmful.

The Ministry of Health also said that there are strong medical protocols in place at all vaccination sites to ensure the safety of those who received the vaccine.
