The plan mentioned the need to carry out patient assessment and screening and preventive intervention services.

  The plan requires that primary medical and health institutions, elderly care institutions, and medical and elderly care institutions conduct regular cognitive function assessments for the elderly.

When implementing the National Basic Public Health Service Elderly Health Management Service Project, primary medical and health institutions use AD8 and the Concise Community Dementia Screening Scale to evaluate the cognitive function of the elderly in their jurisdiction in conjunction with the health examination of the elderly.

Aged care institutions and integrated medical care institutions shall regularly assess the cognitive function of the elderly in the institutions.

For the elderly who are suspected of dementia, it is recommended that they go to a higher-level medical institution for treatment.

The screening rate of cognitive function in the community (village) elderly over 65 years old reaches 80%.

  The plan requires the development of preventive intervention services.

Relying on the medical consortium, etc., the grassroots dementia prevention and intervention services are carried out, and targeted treatments are carried out for the elderly with different degrees of disease.

Establish and improve the Alzheimer’s prevention and treatment service network, and establish and improve the prevention and intervention model that combines patient self-management, family management, community management, and hospital management.

  The plan has determined the work targets for the pilot areas by 2022, including 80% of the public's awareness of the prevention and treatment of Alzheimer's disease and an 80% screening rate for the cognitive function of the elderly in the community (village).
