Spain does not have herd immunity, "nor will there be", according to the Minister of Health, Salvador Illa. Only 5% of Spaniards have antibodies against the coronavirus, far from the 60% intended. This leaves the country in a delicate situation at the risk of a Covid-19 outbreak. "The risk of an impact similar to that which we have had in many parts of Spain is possible," Fernando Simón warned yesterday. The wall to stop the return of the virus is the "extreme vigilance" of the infected and their contacts. A task that falls on the communities, specifically in the departments of Primary Care and Public Health.

The autonomies have been forced this week to foresee and implement epidemiological surveillance plans and tracking contacts against the clock. Creation of teams and contracts to build an army of more than 2,000 trackers, responsible for following and monitoring all people who come into contact with a person infected with the aim of preventing a rebound in the virus.

Madrid (400) . The use of health personnel and the creation of a team of trackers has been used before Health as one of the main arguments to move to phase 1. Specifically, the Community plans to hire 400 Public Health technicians who will perform the functions of "trackers "to identify close contacts of those infected with coronavirus and monitor them daily. They will analyze "environments" and "family". In addition, the Ministry of Health explains that it intends to hire 772 health workers, 650 of them for Primary Care.

Catalonia (200) . To carry out early detection of those infected and follow-up on their contacts, the Generalitat Health Minister, Alba Vergés , anticipated the hiring of 200 trackers to reinforce the detection system.

Galicia (50) . The Xunta has launched this week a technological platform to track and monitor the contacts of infected people. It has been launched with an initial team of 20 operators, but they hope to reach 50 in the coming weeks. When a doctor diagnoses a person as a Covid-19 suspect, they are asked for the details of people with whom they have been in close contact, from two days before the onset of symptoms to the time of isolation. The epidemiological surveillance system will identify and maintain direct contact with these people. Public Health and Preventive Medicine technicians collaborate on the device.

Valencia (270) . In its first attempt, the Valencian Community failed in its intention to go to phase 1. In the report sent to Health, it is detailed that the number of professionals dedicated exclusively to epidemiological surveillance had gone from 66 (12 in central services and 44 in Public Health Centers of the Health Department) in February 2020, to 270 (92 in central services and 178 in Public Health Centers of the Health Department) in the month of April 2020. Of course, the report recognized that only Close contacts were identified in 17.73% of the cases, and of the contacts, only 36.5% became confirmed cases during follow-up.

Asturias (30) . The Ministry of Health will monitor daily the registry of suspected cases and their contacts. To this end, it has set up a Contact Surveillance System , made up of a multidisciplinary group of 30 counseling professionals, to which last-year resident nurses from the specialty of family and community medicine will join. When a positive is identified and confirmed in Primary Care, the information is transferred to this special team to begin the study of their environment.

Castilla y León (61) . The Board has opted for a containment strategy in the request for de-escalation. In its Transition Strategy for Descaling in Basic Zones, it states that the Epidemiological Surveillance Network of Castilla y León has 61 professionals, including senior, media and administrative technicians. The government, however, estimates that the toilets located in the health centers, dedicated to case notification and contact tracking, should not be less than 500 people.

Castilla-La Mancha (427) . This week, the strategy for monitoring and controlling cases of COVID patients and their contacts began, in charge of the Nurse Investigators of Cases from Primary Care, due to the citizen's proximity to this type of health resources. The Board has created a team made up of more than 427 nurses, whose main task is to notify the case, carry out the epidemiological survey, investigate contacts and follow-up and monitor them, in the quotas of more than 1,000 health cards. existing in the region.

Restrooms from a Barcelona health center prepare the material for home visits.

Cantabria (47) . "It is as important to keep positive cases under control as it is to be able to make an early diagnosis of symptomatic cases," points out the Minister of Health, Miguel Rodríguez . The data provided by the Cantabrian government is that it has expanded to 20 professionals, eight more than those used in the flu campaign, the network of sentinel doctors of Covid-19 in Primary Care. In addition, it is planned to increase the staff of the General Directorate of Public Health with 10 new technicians, bringing the number of professionals dedicated to the surveillance of the coronavirus to 47.

Balearic Islands (90) . The Ministry of Health and Consumption has launched this week the Strategy for diagnosis, surveillance and control of the transition phase of the pandemic generated by Covidien-19. According to the information of the Ministry, the contact study of the four islands is centralized in Mallorca and will be carried out by primary care nursing personnel. The forecast is to add the team to be made up of more than 45 nurses, in addition to another 45 auxiliaries, as reported by Hora Hora.

Extremadura (130) . In this community, Public Health is integrated into Primary Care, so that in Health centers, apart from the activity of public health doctors and nurses, they have Pharmacists and Veterinarians. Thus, the investigation of the contacts is carried out in each health area by the pharmacists or veterinarians and the monitoring and control of the contacts by the doctors and nurses of Primary Health Care. In the Cáceres health area , 58 staff professionals work directly in this surveillance; and in Badajoz , 72.

Canary Islands (40) . Being islands and having different realities, two tracking methods have been established. On the one hand, a group of 30 toilets specializing in tracking infections has been created in the Primary Care Management of Gran Canaria. They work from an external operating post installed in the State Public Library (in Las Palmas ) from where they centralize the data that comes to them from health centers. On the other hand, in the Primary Care Management of Tenerife and in the rest of the non-capital islands, the screening is done with family doctors and nurses who are assigned a quota of patients. On this total, a screen is made to narrow the search according to the data collected and the results are passed to the verification team made up of 11 toilets.

Murcia (10) . The Murcian government considers that the 10 trackers that its health system has are sufficient, since, according to data from the Ministry of Health, during these last two months of pandemic they have followed up on 60,000 people. However, they consider the option of hiring if the situation requires it.

Andalusia (450) . Around 450 professionals, according to the data offered by the Board, make up the Epidemiological Alert Network , which works daily in a coordinated way to monitor the numbers associated with the virus and try to stop transmission.

La Rioja (100) . It has established teams with nursing professionals, around a hundred, whose job is to trace close contacts. Its objective, as Alberto Lafuente explains , is an "active search" of these contagious contacts. "These teams are going to have the most important role in de-escalation," which defines Sherlock Holmes as a task .
