  • It will be deployed in Gironde and Dordogne in the spring, after a successful test in the Landes.

Identifying symptoms earlier to improve the management of certain chronic inflammatory diseases is the challenge for the Guardian Angel application. "The special feature of this application is that it is aimed at both the patient and the doctor, whereas in general, there are on the market either decision-making applications for the doctor or e- coaching for the patient. Here we mix the two approaches ”, explains Ulysse Moutard, director of health at Capgemini, the company based in Mérignac near Bordeaux, which developed“ Guardian Angel ”.

“The Region is providing a large subsidy of 1.5 million euros and has supported the system against all odds for two to three years, notes Françoise Jeanson, regional councilor of New Aquitaine delegated to Health. We are accompanying its launch and it will then have to find its own model ”.

Application will have a stronger effect on medical deserts

Depending on their specialties, the professors behind the project - Patrick Berger, Jean-Luc Pellegrin, Thierry Schlaeverdeke and Patrick Blanco - decided to focus on four chronic pathologies: asthma, chronic obstructive pulmonary disease (COPD), diabetes and Raynaud's phenomenon. First tested in the Landes with 200 patients and 80 general practitioners, the application will be extended from April to the departments of Gironde and Dordogne.

“These pathologies are related to relative emergencies, in the sense that we do not speak of cerebrovascular accident (stroke) for example. Some of these diseases develop six months after the first symptoms, explains Professor Patrick Blanco, head of the immunology laboratory at the Bordeaux University Hospital. And the idea is to work with general practitioners to optimize the care of the patients concerned, in particular in medical deserts. ” If the application, and more generally the service, targets both urban and rural areas, "tests have shown that it may have a stronger impact in rural areas where access to a specialist is more complicated", specifies Ulysse Moutard.

On Raynaud's phenomenon, which affects 10 to 15% of the population, "it is generally mild but sometimes it can be a harbinger of a serious illness," explains Patrick Blanco. Our ambition is also to capture rare diseases and avoid being at a stage where we are beyond therapeutic resources ”.

What follow-up for the patient?

Depending on a biological or clinical symptom, the doctor will suggest that the patient benefit from the Guardian Angel device. If he accepts, he will have access to personalized recommendations, to follow-up care via health indicators shared with his (or her) doctor (s). He may also receive notifications related to his treatment path, his medication, etc. The application also offers secure prescriptions and an action plan in the event of a crisis.

"The medical information entered by the patient is shared with his doctor, via his own version of the application, so that the consultation is enriched with data for better exchange and better decisions", points Ulysse Moutard.

"The application also serves research in primary care, since the patients treated at the CHU are not representative of the pathologies of the general population," notes Professor Patrick Blanco. He adds that for the patients followed within the framework of this device, the environmental aspects (air pollution, exposure to an agricultural activity, etc.) can also be appropriately studied.

After a few months of experimentation from April, an increase in the number of patients monitored is expected in September and should bring the cohort beyond 10,000 people.
