Over 90% of the burden will be paid by people under 74, including the working generation. In principle, the burden required for members of the "Late-stage Elderly Medical Care System" who are 75 years of age or older will be 8%.

This is based on the current medical insurance premium burden ratio. In order to secure financial resources to combat the declining birthrate, the government has created a "support subsidy system" and plans to start operating it from fiscal 2026.
