Maeda returned to the big leagues for the first time in two seasons after undergoing Tommy John surgery to repair a ligament in his right elbow in September 2021. He started against Masanao Yoshida's Red Sox in search of his first win after returning from right elbow surgery.

But he walked off the mound in the second inning after a hard hit on his foot. The Red Sox won the game 1-1, and Maeda was the losing pitcher, ending a three-game losing streak from the start of the season.
