Question : Qian Suyun: In "Pediatrics", the university asks the speed of life and death to protect life

  Reporter Du Yan

"The child's condition is changing rapidly, life and death are at stake, and delays or mistakes cannot be tolerated." According to Qian Suyun, honorary director of the Department of Critical Care Medicine (PICU) and the Department of Internal Medicine, Beijing Children's Hospital, pediatrics is also a university study and a university department.

Only by working hard, not slack, and not giving up, can we live up to the heavy trust.

Qian Suyun, who is of medium stature and has short hair, always has a pair of crescent-like smiling eyes, which makes people feel like a spring breeze.

Perhaps in the face of children every day, she is always gentle and patient with others, but has a core of courage and tenacity.

She has been in pediatrics for nearly 40 years, and has been in the field of critical medicine for 26 years. She has saved countless children who were dying and helped them win wars without gunpowder smoke.

Live up to the heavy trust and keep the hope of life for the children

Qian Suyun can be described as "the second generation of doctors".

Influenced by her mother as a medical staff, in 1979, she entered Taishan Medical College (now Shandong No. 1 Medical University) to study and graduated with honors. Later, she took the initiative to enter the pediatrics department, hoping to help sick children relieve the pain and keep hope for the family. .

In his spare time, Qian Suyun takes time out every day to study hard and improve himself to help more children.

The hard work pays off. In 1993, Qian Suyun was admitted to the Capital Medical University for postgraduate studies with excellent grades.

In 1996, Qian Suyun officially entered the Beijing Children's Hospital to engage in the work of critical care medicine for children.

A 6-year-old child was sent to the emergency department due to abdominal pain. The child had sudden convulsions after the infusion, and was unable to maintain spontaneous breathing. He needed a ventilator to maintain his life.

Everyone speculated that it might be abdominal disease, but Qian Suyun suspected that it was a brain problem, and proposed to do a brain CT to confirm the diagnosis.

At that time, the hospital did not have a transport ventilator specially used for the transfer of patients, which meant that the child had to be temporarily removed from the ventilator, which was a risk, and the parents were firmly opposed.

However, in order to make a clear diagnosis, Qian Suyun firmly said: "We have to take this risk. We can use the resuscitation bag to connect the tracheal intubation to ventilate the child. If the child has problems during the examination, I will be responsible!" After obtaining the consent of the parents, Qian Suyun and colleagues escorted the child to the CT room together.

She put on a lead coat, and even if she was exposed to a certain amount of radiation, she kept pinching the resuscitation airbag by the CT machine to accompany the child to complete the examination.

The results showed that the child suffered from a brainstem tumor, and the sudden aggravation of the condition was caused by internal hemorrhage of the tumor. This "risky" CT examination confirmed the diagnosis for the child.

In Beijing Children's Hospital, many children come from thousands of miles to seek the last ray of hope.

There was a 4-month-old child who was paralyzed at the time of treatment. The local government considered it to be a congenital disease and told the parents that most of these children had a poor prognosis, and the parents were almost desperate.

When they came to Beijing Children's Hospital, Qian Suyun and her colleagues inquired about the medical history in detail and did a careful physical examination. After accurate examination, it was confirmed that the child had infant botulism poisoning. After standardized treatment, the child recovered well.

"The child's condition is changing rapidly, life and death are at stake, and delays and mistakes cannot be tolerated. Only by working hard, not slack, and not giving up, can you live up to the heavy trust." Qian Suyun often said.

Be a teacher and be a doctor with a clear conscience

"Are you afraid of being tired?"

"Can you fall asleep on your back?"

This is Qian Suyun's "soul torture" of the students.

For nearly 40 years, Qian Suyun always arrived at the hospital just after 7:00 the next morning to start his day's work, even though he was dealing with things late the night before.

Room rounds, rescuing patients, talking with family members, handling teaching and scientific research affairs... Every moment in the hospital is tense.

In Qian Suyun's view, being a qualified pediatric intensive care doctor requires not only medical skills, but also composure and the ability to withstand pressure.

With these "basic skills", we can strive to be a doctor with a clear conscience and reduce pain for children.

"Children's critical care medicine has high risks, high intensity, and limited medical means and capabilities. However, even if you feel helpless and frustrated at work, as a medical staff, you must do your best and do your best to prevent the disease. Children’s hope for life.” Qian Suyun said that many children in the intensive care unit need invasive treatment such as intubation, and medical staff will try to let the children receive treatment in a relatively comfortable and painless environment. May ease the pain of the child.

As a student of Qian Suyun, when the teacher pointed out that he was not in the right place during the ward rounds, Mao Yiyang would "swept up"; when the teacher found out that there were deficiencies in his medical records, Mao Yiyang would be ashamed.

But this made him more firm in his belief: to follow Mr. Qian as an example and be a pure pediatric intensive care doctor with a clear conscience.

Influenced by Qian Suyun, Mao Yiyang hopes that he can help children and families get rid of the pain through professional skills in critical situations: "I only focus on how to make the patient better, and only do what I should do, not because of the pain. I am worried about possible conflicts and disputes that may stymie us.”

Leading disciplines and cultivating outstanding "new forces"

As one of the leaders of pediatric critical care medicine in China, Qian Suyun has been leading and undertaking the important task of promoting the development of pediatric critical care medicine.

Wang Quan, Secretary of the Party Branch of the Emergency Department of Beijing Children's Hospital and Director of the Department of Emergency Internal Medicine, was Qian Suyun's first batch of students and her favorite student.

In Wang Quan's memory, there were only a few people in the first intensive care unit in Beijing Children's Hospital, with only a dozen beds, and ventilators and monitors were already "high-end" equipment.

Wang Quan said that Qian Suyun's resolute and resolute eyes when rescuing patients, his gentle and concise words when giving doctor's orders, his generosity regardless of personal gains and losses when helping others, and his dedication and unreservedness when supporting younger generations, are the face of fame and fortune. His indifference and self-discipline are the "fuel pumps" that make him happy to move forward in pediatrics.

Liu Jun is the deputy director of the Department of Critical Care Medicine at Beijing Children's Hospital. He started working in 2004. At that time, although the Department of Critical Care Medicine had begun to take shape and was leading in China, it was still imperfect in many aspects.

In the past 18 years, Liu Jun has participated in and witnessed the development and growth of the intensive care department led by Qian Suyun.

At present, the number of beds in the intensive care department has increased to more than 50, with multi-function monitors, blood gas analyzers, invasive and non-invasive ventilators, bedside blood purifiers, ECMO, bedside X-ray, bedside color ultrasound, bedside With advanced critical care rescue equipment such as para-EEG, transcranial Doppler ultrasound monitoring, etc., a medical and nursing team with excellent technology, experience and innovative spirit has been formed. It is one of the most comprehensive pediatric critical care medicine departments in China. one.

"Although there is more technical equipment support now, I have always kept Mr. Qian's words in my mind: we should stand by the bed more to observe the children." Liu Jun said that under the guidance of Qian Suyun, the young doctors kept thinking Problems, doing scientific research based on clinical needs, not only better solve the problems encountered in the process of diagnosis and treatment, but also continue to promote the development of the discipline.

The industry is skilled in diligence, and the action is in thinking.

As a doctoral tutor, Qian Suyun is rigorous in his studies.

More than 20 years ago, she began to study the application of transcranial Doppler (TCD) in the monitoring of critically ill children, a field that was still blank in the domestic pediatric community at that time.

After her efforts, she proved for the first time in China that TCD monitoring is helpful for evaluating the prognosis of comatose children and guiding treatment. She also proposed the TCD diagnostic criteria for childhood brain death for the first time and was included in the "China Children's Brain Death Determination Criteria and Operational Guidelines".

The technology has been popularized and applied in PICUs of more than 70 hospitals in China, which can partially replace invasive examinations, improve the accuracy of disease judgment, and reduce medical expenses.

She presided over the formulation of more than 10 guidelines/consensus including "Expert Consensus on Diagnosis and Treatment of Septic Shock (Septic Shock) in Children (2015 Edition)", and participated in "Guidelines for Diagnosis and Treatment of Hand, Foot and Mouth Disease" and "Severe Enterovirus 71 (EV71) Infection" The formulation of the Expert Consensus on Clinical Treatment of Cases and the Diagnosis and Treatment Plan for Novel Coronavirus Pneumonia (Trial Eighth and Ninth Editions).

Qian Suyun has many honors: National March 8 Red Banner Bearer, Capital Labor Medal Winner, Beijing March 8 Red Flag Medal Winner, Beijing Advanced Worker, Capital Spiritual Civilization Construction Award Winner, and the 6th "Top Ten Health in Capital" "Guardian", "China's Good Doctor" Monthly Person, "Excellent Communist Party Member" of Beijing Municipal Health Commission and Beijing Medical Management Center, and CCTV's "Most Beautiful Doctor" honorary title...

"I am a grassroots doctor, and I am honored to be a representative of the 20th National Congress of the Communist Party of China. I feel that I just did what I should do." Qian Suyun said that pediatrics is actually a university department.

With the development of the country, the number of specialized hospitals for children has increased, and the number of pediatric medical workers has also increased.

The pediatric critical care medicine I am engaged in is a profession that requires multidisciplinary collaboration. I hope to promote the development of pediatric critical care medicine through my own efforts, "to help the child at the most critical juncture of his life." (End)
