The scope of establishment includes national demonstration provinces (autonomous regions and municipalities), national demonstration counties (cities, districts), and national demonstration institutions for the integration of medical and elderly care.

The conditions for the establishment of national demonstration institutions for the integration of medical care and elderly care are that they have been in operation for 5 years or more, the occupancy rate in the past 2 years has reached 60% or more of the actual operating beds, and the proportion of disabled and demented elderly people is more than 50%.
Priority is given to recommending institutions that carry out comprehensive health and needs assessments for the elderly, establish electronic health records for the elderly, actively intervene in geriatric syndromes to prevent or alleviate disability and dementia, and focus on combining medical and nursing care with the characteristics and advantages of traditional Chinese medicine.
At the same time, the national demonstration institutions for the integration of medical care and elderly care should meet the requirements of "five good", namely, good environmental facilities, good personnel, good internal management, good service quality, and good service effect.
The national demonstration province (autonomous region, municipality) work flow for the integration of medical care and elderly care is application creation, support and guidance, evaluation and acceptance, and dynamic management.
The work flow of the national demonstration counties (cities, districts) and demonstration institutions for the combination of medical care and elderly care is self-assessment declaration, preliminary review recommendation, evaluation and acceptance, public announcement and naming, and dynamic management.
