The incidents occurred in April when a nurse on her own initiative allegedly gave double doses of morphine to two patients.

Both patients lived in a nursing home in the county and had been prescribed palliative medication by doctors.

After the double doses of morphine, both patients died shortly afterwards.

Both incidents are now reported according to Lex Maria.

According to the report, this has occurred at ten-day intervals and on both occasions the nurse has made the assessment that the patients needed more than the prescribed maximum doses.

However, the nurse has admitted that doctors should have been contacted before the decision.
