According to the municipality, the woman had an installed security alarm but which was not used.

According to the municipality's investigation, the woman was talkable and had the ability to contact authorities, healthcare and SOS alarms when she was discharged from a treatment home.

Granted home care

The woman had been granted home care 15 hours a week after being discharged from her treatment home.

But at the time of discharge, some kind of communication error arose between the municipality's client and the performing home service, it is stated in the municipality's internal documents that SVT has read.

The investigation states that one of the explanations is judged to be that "the contractor did not read through the entire document to make sure from when the order was valid and its scope".

The incident was reported

The incident is considered serious and has therefore been reported to the Swedish Health and Care Inspectorate, IVO.

In total, it is about 47 days that the woman has gone without a visit from the home care service until she was found dead in her home, according to a report to IVO.

Haninge municipality does not want to comment.
