Medical care is the need for people's livelihood, and medical and health care is related to the health and happiness of hundreds of millions of people.

Every year at the National Two Sessions, the medical and health field is the focus of the delegates.

This year, the representatives and committee members offered suggestions on strengthening the construction of primary medical institutions, helping the elderly to seek medical treatment through technology, and improving the nursing practice system.

Strengthen the construction of primary medical institutions

"In recent years, the rural cancer screening and early diagnosis project has relied on primary medical institutions, and a total of 215,000 cancer patients have been found." On March 7, at the second "member channel" of the fifth session of the 13th National Committee of the Chinese People's Political Consultative Conference Wang Guiqi, a member of the Chinese People's Political Consultative Conference and director of the Endoscopy Department of the Cancer Hospital of the Chinese Academy of Medical Sciences, mentioned such a data.

This not only shows the necessity of moving forward on the prevention of major diseases, but also reflects the importance of primary medical institutions to disease prevention and control.

Statistics show that by the end of November 2021, there were 1.044 million medical and health institutions nationwide, including 990,000 primary medical and health institutions.

Improving grassroots disease prevention and treatment capabilities has also been written into this year's government work report.

"Grassroots medical institutions are responsible for the prevention and control of common diseases, frequently-occurring diseases, chronic diseases and public health of rural residents, as well as tasks such as epidemic prevention and control, publicity and education, and are the terminals of my country's tertiary medical and health network." Geng Funeng, chairman of Doctor Pharmaceutical Group, believes that "strengthening the grassroots" is the necessary foundation to promote the high-quality development of medical services, help rural revitalization, and promote the construction of a healthy China.

In recent years, my country's primary medical care has made great progress, but Geng Funeng found in his research that there are still weak links in some basic fields.

For example, some clinics and clinics in remote areas have problems such as insufficient basic hardware conditions, lack of basic medical facilities, and insufficient types of medicines.

At the same time, the rural doctor team has the current situation of low education, aging talent structure, low level of professional knowledge and skills, and skills allocation that cannot meet clinical needs.

"To improve the ability of grass-roots disease prevention, treatment and health management, the key is to upgrade the supporting equipment of grass-roots medical institutions, improve the hardware and software conditions, improve the diagnosis and treatment capabilities of rural doctors, and improve the structure of grass-roots medical personnel." Geng Funeng suggested that the investment of resources should be guaranteed and scientific and reasonable. Configuration, strengthen the technical training of common diseases and frequently-occurring diseases of rural doctors.

Zhou Songbo, a deputy to the National People's Congress who has been rooted at the grassroots level for a long time and a doctor in the clinic of Diaosi Village, Zhuozhou City, Hebei Province, has personal experience with the problems of lack of software and hardware and insufficient service capabilities exposed by grassroots medical institutions.

How to change this status quo?

Zhou Songbo believes that it is necessary to further increase investment in the construction of rural health service system, continue to strengthen the construction of business premises and auxiliary facilities of rural medical and health institutions, and ensure that the national and provincial standards are met.

At the same time, increase investment in equipment equipment and update, and improve basic diagnosis and treatment capabilities.

In addition, it is necessary to strengthen the information construction of rural medical and health institutions, and improve the efficiency of diagnosis and treatment services and public health services.

Rural doctors are the "gatekeepers" of the health of the grassroots.

Constrained by difficult conditions, insufficient treatment, and limited room for promotion, there are problems such as talent shortage, large age structure, and the alternation of old and new in rural doctors in some areas.

"Further enriching the team of primary medical personnel is an important means to meet the needs of the masses for medical treatment and improve the capacity of primary medical services." Hu Wanning, member of the National Committee of the Chinese People's Political Consultative Conference and president of the Workers' Hospital of Tangshan City, Hebei Province suggested that while improving the treatment of primary medical personnel, use Make good use of the high-quality medical resources of higher-level hospitals, improve the reward and promotion mechanism, and further create a good development platform for them.

At the same time, it helps them solve the problems they encounter in study, work and life, and relieves them from the worries of staying at the grassroots level.

Applying information technology to help medical treatment

According to China's economic data for 2021 released by the National Bureau of Statistics, there are about 200 million people aged 65 and above in my country, accounting for 14.2% of the country's total population.

This year's government work report also proposes to make up for shortcomings in services such as geriatric medicine.

Xiao Youxiang, deputy to the National People's Congress and secretary of the Party branch of Qingfeng Village, Le'an Town, Anhua County, Hunan Province, noticed the difficulty in seeking medical care brought about by the aging population.

She suggested using information technology to establish a remote consultation platform in primary medical institutions, opening up service channels for hospitals and medical experts in large cities to conduct remote consultation for grassroots patients, and using telemedicine to solve the "last mile" for the elderly to seek medical treatment.

Liu Qingfeng, deputy to the National People's Congress and chairman of iFLYTEK, paid attention to the chronic diseases caused by aging.

He believes that with the accelerated aging of the population and the rising proportion of elderly patients with chronic diseases, information technology should be used to promote chronic disease management services to benefit more chronic disease groups.

"The application of artificial intelligence technology in primary medical care provides new ideas for solving chronic disease management problems in my country's aging society." Liu Qingfeng suggested that the National Health Commission should incorporate intelligent voice outbound calls as a public health service method into the "National Basic Public Health". "Service Specifications", when the effect of the machine reaches or exceeds the human level, it can be included in the total work of family doctors like human services to solve problems such as the shortage of basic public health service personnel.

In addition, consideration should be given to incorporating AI-based grassroots refined chronic disease management services into the medical insurance fee catalog to further encourage the large-scale application of AI-based chronic disease management models.

  Improve the nursing practice system

Faced with an aging population, Li Xiuhua, member of the National Committee of the Chinese People's Political Consultative Conference and honorary chairman of the Chinese Nursing Association, is more worried about the shortage of nursing staff.

According to data from the National Bureau of Statistics, by 2021, the number of registered nurses in my country will reach 5.02 million, but the number of nurses per 1,000 people is still low.

How to change the current situation of this contradiction between supply and demand?

Li Xiuhua suggested that the Nursing Law be introduced as soon as possible.

"The Nursing Regulations currently implemented in my country are administrative regulations, which are weak in terms of mandatory and binding protection of nurses' rights." Li Xiuhua believes that the introduction of the Nursing Law can protect the legitimate rights and interests of nurses from the legal system level and strengthen the construction of the nurse team.

Hu Chunlian, deputy to the National People's Congress and director of the outpatient department of Yueyang People's Hospital in Hunan Province, also led the submission of a motion on accelerating the formulation of the Nursing Law this year.

She added that the introduction of the Nursing Law will not only help to enhance the society's respect for nurses, but also help solve practical problems in nursing work.

For example, to further clarify the basic business scope and prescription authority that need to be defined in the development of nursing, as well as the access conditions, scope of practice and management rules of various specialized nurses.

When it comes to nursing work, nursing workers are another group that cannot be avoided. Controversy has always been around the issue of “retention” of nursing workers.

According to Zhu Tongyu, a member of the National Committee of the Chinese People's Political Consultative Conference and vice president of Zhongshan Hospital Affiliated to Fudan University, with the aging of our country's population, the contradiction between medical care and life care needs is prominent. Extremely prominent, reflecting the urgent need to improve the nursing system in our country.

Regarding the nursing group, Zhu Tongyu found in the survey that most nursing staff are currently employed by patients and their families, and medical institutions lack effective supervision of nursing practice, resulting in frequent occurrences of disorder such as arbitrary charges and competition for patients.

Due to the lack of access standards for nursing workers and the lack of standardized training, most of the nursing workers are employed with "zero experience."

In this regard, Zhu Tongyu suggested that the construction of the nursing practice system should be improved. According to different work contents and responsibilities, it is recommended that the nursing system be divided into different levels such as nursing assistants, assistant nursing, medical nursing and nursing scientific research. The difficulty and risk are determined by different personnel. undertake and form a regulatory system.

"It is necessary to implement the unified employment and management system of nurses by medical institutions, learn from the international common practice of 'zero escort', take life care as part of the patient's recovery, and hand it over to the hospital for unified management, so as to fundamentally prevent nurses from being managed by a third party. It is necessary to establish a training and use system for medical nurses in my country, improve the professional certification system for medical nurses, formulate a reasonable nursing price system, and incorporate part of the cost of living care into the medical insurance system. Reasonable sharing of costs by society and families.
