at the gates of Andalusian health centers.

Complaints from users about not making an appointment with their GP quickly have also been frequent.

Health professionals have also

demanding more resources.

With the new consultation model, the Board intends to better organize patient care.

Despite the criticism, the Minister of Health,

, has defended this model and has served as the basis for the measures that the Ministry of Health is designing with the autonomous communities to address the

The new reception consultations were implemented in an experimental way in 11 of the 34 health districts of Andalusia.

Currently, they are already operating, according to the Board, in about 400 centers, 90% of the primary care network.

According to the data collected in the first districts that adopted these new consultations, 55% of the cases were attended to satisfactorily by the nurses. In other words, the patients who went to the health centers without an appointment left with the problem solved, without the need for a doctor to see them. In addition, of all the patients who attended, less than 4% were immediately referred to the

emergency room

because they suffered from some pathology that required treatment that same day.

Nurses can resolve up to

18 healthcare processes

previously agreed with scientific societies and professional associations.

The pathologies that they can treat range from anxiety, vomiting or diarrhea to the dispensing of the morning after pill, an emergency method to prevent pregnancy, which is used when a woman had unprotected sex or when the contraceptive method failed. you used.

Below are the 18 protocols that regulate the way nurses attend to the different pathologies in the new 'reception' consultations.

Morning after pill

The nurses attend to the patient who requires it when she has had

unprotected sex

and there is a risk of pregnancy; if there were errors in contraception, such as incorrect use or lack of adherence to hormonal treatment; delay in taking oral contraceptives; delay in the application of injectable contraceptives; condom breakage during intercourse; or due to the expulsion of the IUD.

Among the criteria for excluding patients who attend these new consultations is that they be

under 16 years of age

; if there was a sexual assault; when there are liver pathologies, inflammatory bowel disease (Crohn's) and a history of ectopic pregnancy. Other exclusion criteria is that the woman is already pregnant or that more than five days of unprotected or risky sexual intercourse have passed. If the nurse or the patient so considers it, they can also be excluded from this consultation.

The protocol states that, in addition to dispensing the morning-after pill to the patient, the nurse must explain the available emergency contraception options, describe the

and that it is an emergency contraceptive method, which should not be substituted to a method of regular use.

Anxiety

The protocol that regulates new consultations defines anxiety as "an emotional state, in anticipation of future harm or misfortune, accompanied by somatic feelings of tension and dysphoria".

Among the criteria for inclusion of the patient in the consultation attended by the nurse is having a feeling of suffocation or shortness of breath associated with two or more of the following symptoms: "

sweating, chills or flushing

, nausea or abdominal discomfort, feeling of choking, dry mouth, trembling or shaking, paraesthesia and / or tingling, headache, instability or dizziness, increased heart rate, muscle tension, trouble falling asleep, difficulty concentrating and feeling of memory loss, fear of losing heart control and / or die ".

After a first assessment by the nurse, the patient will be referred to a medical consultation when they are

under 14 years of age

, whether they are pregnant, there is a previous Mental Health diagnosis (serious mental disorder), they have suicidal ideas, they are drug users, drugs or alcohol, or are vomiting.

Allergic reactions

It is another ailment that nurses can attend to in new consultations.

They may respond to cases of

They should not treat all patients who show allergic reactions.

The protocols on new consultations attended by nurses in primary care also include care for patients suffering
