Most homes that are inspected by Ivo, the Swedish Health and Care Inspectorate, are well maintained and receive no criticism or have minor problems to deal with.  

But the criticism and events at the HVB homes listed below come from supervision protocols, judgments and reports from the businesses themselves.

They are picked because they have received repeated, and severe criticism.

Some have been closed down in 2020, but are involved because the municipalities have placed children there for the past three years despite the criticism.

We have updated the list with the whole of 2020 and the first months of 2021.

Active HVB Girls, Åtvidaberg 

2021  

The home is criticized by Ivo for suitability assessments that need to be improved and for staff not complying with the law's requirement to report misconduct under the Lex Sarah Act.

One girl states that unauthorized people sneaked in from the balcony at night, that there were drugs and that it was "messy." 

2020 

The activity does not provide conditions for the young people to be safe, Ivo assesses.

At the time of the inspection, two girls had recently attempted suicide and one of these girls had expressed suicidal thoughts.

A previously enrolled girl made repeated suicide attempts, among other things, by swallowing pieces of glass.

The events described show the problems that arise when homes receive young people with problems that they do not have permission to care for, writes Ivo. 

Conflicts have arisen among the young people who are escalating and a young person is setting fire to the room.

The fire is extinguished by personnel.

Security company, police come to the business. 

2019 

Continued difficulties in meeting the young people individually and stress extracts are not made in accordance with current regulations.

The home has received a young person, not documented suitability assessments and has not written an enrollment decision. 

2018 

The business does not take register extracts to check staff, urine samples are taken regularly, impermissible rules of procedure.

The activity has difficulty assessing the young people individually. 

2017 

A girl is raped in a home for which Active Care is responsible. 

The report is that a girl has taken most of the tablets that she has saved for a while. 

A girl has been given the wrong medicine.

The cause of the malpractice has been identified as two clients sharing the same medicine cabinet. 

Mårten Bjurströmer, CEO, answers SVT: 

“We have revised and improved our registration procedures at enrollment. IVO takes no action, but follows up at the next inspection. Regarding that the staff must be aware of their reporting obligation according to Lex Sarah. Active Omsorg conducts systematic quality work and has clear routines regarding, among other things, deviations and Lex Sarah. During introduction and continuous during employment, the staff is trained in our routines, it is part of our systematic quality work. In the last two years, we have strengthened our training efforts regarding, among other things, deviations and Lex Sarah. During the summer of 2020, IVO carried out an inspection of Active HVB Sandgatan based on complaints received. IVO decides to close the case as they did not notice any shortcomings in the supervision.IVO has chosen to close all cases cases concerning previous deficiencies linked to inspections made during 2017-2020. " 

The treatment home Källtorp HVB, Hova

2020

Ivo continues to criticize shortcomings in the staff's overall competence.

Seven out of 17 employees are permanent employees.

Of the 10 fixed-term and hourly employees, none have at least two years of post-secondary education with a focus on social work, social pedagogy or behavioral science.

Of the seven permanent employees, two have at least this education. 

A previously enrolled girl has been sexually abused on the premises of the business.

2019

Insecure children, "a couple of the girls" exposed to violence from young people, and inappropriate behavior from a staff.

Deficiencies in security and safety for the enrolled children and in the staff's skills. 

2017

Young people have taken drugs and also given other young people.

Another youth abused the drug.

IVO notes that it has been seven months before the investigation began.

The principal has breached its obligation to report.

A placed girl was abused by another placed youth.

She was given drugs by the boy and they had sex.

It took eight months before the investigation was started.

- We have made an analysis of the criticism, we do something about the criticism and change a management team.

We provide new conditions, which we are very proud of.

Bills Bo- och Utbildningsgård, Tierp

2020

The operation has not been able to ensure safety and security for enrolled young people.

They have not perceived their responsibility as caregivers.

Young people have been threatening and aggressive and there has been violence and gross destruction.

Both young people and staff have been afraid of certain enrolled young people.

Staff have been forced to lock themselves in to protect themselves.

During a weekend, everything escalated and the situation became so unsustainable that the Billsbo unit was closed.  

At the Lillsbo unit, planned treatments have not always been carried out, something that IVO pointed out during two previous inspection visits.

Young people state that you have to "tease the staff" to get their calls.  

The principal's statement to IVO states that the business, with the units Billsbo and Lillsbo, has been temporarily closed down and that the last young people were discharged on 31 January 2020.  

2019 

The children do not feel safe, there are shortcomings in the staff's treatment.

Chaos in the unit and staff have resigned, say the eight young people they interviewed.

A girl with a stomach ache was rushed to the emergency room by the guard in the evening due to staff not having acted before.

Had urinary tract infection.

Freezing and bullying.

Drug-affected young people.  

In connection with an unannounced inspection by IVO, two registered girls made an oral report.

In both cases, it is a matter of compulsory medication.   

Lex Sarah report: Girl beat an employee, threatened with more violence against employee and vandalism.

The unit was temporarily closed and IVO thinks they have now done enough.  

2018  

IVO finds shortcomings in terms of compliance with the permit and that the business has not been conducted with good quality.

“The operation has not been able to maintain safety and security for enrolled children with reference to the unplanned discharges that have occurred in a short time and on a large scale.

"IVO has also had reason to question whether the staff who perform treatment elements according to CBT (cognitive behavioral therapy) have training for this and whether the activities have working methods that are relevant in relation to the target group for which the home is intended."

Brännagården HVB, Åre

2019

Quality work is lacking, efforts are not individually tailored, young people's freedoms and rights are limited.

The company is fined to ensure that person X is not allowed to work alone (as he barely speaks Swedish), that staffing increases and documentation is improved.

2018

Receives criticism that the activities at the inspection did not have written routines for a preventive approach.

In the autumn of 2020, Ivo decided to revoke the permit for the company after SVT's review.

Christian Möller is the CEO of the company, which also runs Vison's halfway house.

We have made mistakes but it feels like a shitty mistake, he says to SVT.

A girl is convicted of taking drugs at home. 

2020  

Ivo criticizes the home for having staff who do not meet the needs of young people, and for not having made sufficient suitability assessments. 

IVO assesses that this may have contributed to a breakdown in the care of a girl who then had to be discharged unplanned.  

2019  

"Quality deficiencies mean that children and young people do not receive the care they need," writes Ivo.

Here, the inspections show that the home was chaotic, with large turnover of staff, lack of skills and lack of respect.  

When the boss was not there, the staff could sit in the office and play computer games, sometimes loud all night so they could not sleep, the children said.  

The children also state that staff violate and threaten them, and that they are prevented from going to school.  

Marie Björkman, operations manager at Caremore, answers SVT: 

“We follow current legislation to ensure, based on this, that drugs do not occur in our homes.  

It happens that the needs presented at a placement later may need to be reconsidered.

If we discover that the problem is of a different nature than what we may encounter, it is our responsibility to reconsider the placement.

To strengthen the written placement assessments, we have made efforts by conducting in-depth training in social documentation.

We have thus strengthened our routines in accordance with IVO's requirements.

IVO then wrote off the case after reported measures. "

Hyttan HVB, Hedemora

2020  

Ivo criticizes the home for having received a girl without investigating what she needs for care or if the care can be given in safe and secure forms.

A girl says that it was "messy" and that there were drugs in the accommodation.

The manager says that it is not true that it was alcohol that was delivered to the accommodation by a man in a car.

Five children write to Ivo that they only feel safe sometimes, a girl that she never felt safe in the accommodation. 

2019

Children feel insecure after threats and abuse from other young people and the children have "had to intervene themselves when a quarrel arose", writes Ivo.

The staff takes mobile phones and computers from the children without asking and has poor routines for enrollment.

In addition, girls have reported being scared because they were threatened and staff were sexist and abusive.

After that, a person from the management shouted at the children and threatened to send them to a SiS home, they tell Ivo's inspector.  

To Ivo, they state that they have not been shouted or threatened, that the staff is competent and that they have gone to the bottom of the problems.  

Franz Agefjäll Baumann, CEO Curmitis Gruppen responds to SVT: 

“Everyday life includes dealing with girls who are sensitive to social correction, have a trust problem (suspicion), aggression problems and impulsivity.

Every decision made about the residents is based on a risk and maturity assessment, according to a recommendation from the National Board of Health and Welfare, which includes guardians, social services and the treatment home.

All important decisions are discussed and anchored with our legal doctor who is a child and adolescent specialist in psychiatry and with our legal psychologist who is a clinical specialist and legal psychotherapist.

These are done to ensure protection for the child and cohabitants.

All this is communicated / explained and then communicated to the youth. ” 

Hoppet Haga, Heby

2020

Young people have been subjected to inadequate treatment by staff. 

A young person takes an overdose at home, the home writes in a Lex Sarah report. 

2019

Ivo receives reports of threats and abuse at home.

During an unannounced inspection, the young people show mobile videos where the staff is threatening.  

One youth says that one of the staff should say that he should "break his neck".

Here, the home has "failed to maintain a good response", according to Ivo.

They have also used collective punishments, such as refusing young people mobile phones or planned trips home.

Staff must also have refused to give them food.  

Emelie Bonin at Hoppetgruppen writes that they refuse to comment. 

HVB children and young people Duvgården, Heby

2020

The operation has not checked extracts from the Police Authority's suspicion and indictment register before employment.

The documentation of suitability assessments when deciding on enrollment lacks information.

The activities have not adapted the staffing in the home to the needs of the enrolled young people.

2019

Insecurity, drugs, collective punishment, staff shortages and skills shortages. 

Lack of enrollment routines, ie for not having investigated what the young people who are enrolled need for help and how they should work together with the others.

The director, Hatim, answers SVT Nyheter that the criticism was justified and that the routines have now changed.

“My view in this is that the emergence of these shortcomings is a matter of competence.

You simply did not have that competence in place ".

Platea HVB, Hagfors

The children are subjected to coercive and restrictive measures and the home is again criticized by Ivo because "children have repeatedly been subjected to restraints and closures".

In addition, the criminal record for staff has not been checked, and there are shortcomings in suitability assessments and in quality.  

2019

The home has shortcomings in suitability assessments when enrolling for the third check in a row, writes Ivo.

There have also been repeated closures of children, which has aroused fear among the group of children between the ages of 8 and 12.

The children say that then you get no evening coffee.

The principal believes that food has not been withdrawn and that the children have invented "stories", but confirms that closures have occurred "in extreme situations".

2018

The home does not follow the condition regarding the target group, a 5-year-old was placed in the home for one year but such small children may only be admitted urgently for a maximum of two weeks.

Enrollment decisions must be improved, writes Ivo.

The business has also on several occasions closed down and held a child.

- I wish I could say that it is not true.

But it does.

We have worked hard to overcome problems and challenges for a long time, says Platea's CEO Johanna Rastad to SVT. 

Mahults manor HVB, Halmstad

2020  

The documentation of suitability assessments needs to be improved, as there was, among other things, no assessment of how the different placed children can influence each other.

On the other hand, IVO assesses that the staff's competence is sufficient and that the children feel safe and involved.  

2019  

Both several girls who previously lived at home and several of the staff have called Ivo and reported malpractice.

They want to remain anonymous because they are afraid of retaliation.  

Ivo's telephone service notes that "the caller says that those registered are not treated well in the HVB home".  

When Ivo's inspectors go there, they find prostitution, bullying and abuse.

According to the report, the home "does not provide safe and secure care" and the staff have unauthorized searches of the young people's rooms.

 “We take any shortcomings in our operations seriously.

We have rectified the deficiencies found during the inspection in 2019, these were approved in full by Ivo and carried out in the autumn of 2019. In March 2020, Ivo carried out a new inspection which found that the deficiencies had been rectified. ”

Mölnbacka youth home, Deje

2020

Report to the Swedish Work Environment Authority about physical violence.

“The youth then physically attacked staff with blows and kicks.

The staff has averted the situation by holding the youth and laying the youth on the floor.

According to routine and law, staff are not allowed to hold young people, so the staff released the youth on most occasions, which in several cases resulted in the youth continuing to kick and hit staff. ” 

2019

Here, Ivo found serious shortcomings in the business, mainly with illegal coercion and restrictions.

These are closures and other physical interventions and room and body visits.  

Personalen ska ha tagit stryptag, sparkat och spottat berättar ungdomarna. De ska också ha varit hårdhänta och kränkande. Detta nekar personalen till, men Ivo bedömer att ungdomarnas vittnesmål samt journalerna bevisar att man har brottat ner ungdomarna och begränsat dem på olagliga sätt.

”Av beslutet som fattat i februari 2020 anser Ivo att de åtgärder som vidtagits är tillräckliga. Därefter har vi, den nya ledningen, jobbat intensivt med att ytterligare förbättra detta arbete.”

– Min bedömning av verksamheten som är idag är att det är en väldigt god verksamhet, säger Helena Åkerblad.  

Prompting Cornelia och Felicia HVB, Hedemora

Hemmet får kritik för att man inte genomfört den behandling som en flicka skulle ha fått, eller dokumenterat någon uppföljning. Gällande tryggheten uppger tre av flickorna till Ivo att de känner sig otrygga ibland, och två att de inte alltid är trygga från kränkningar som våld eller övergrepp. En tjej säger att det finns mobbning på boendet.

2020

Risk för att en person har utnyttjat en boende sexuellt. “Risk för psykiska och fysiska men som kan kräva större vårdinsatser” står det i Lex-Sarah-anmälan.

2019

Ingen behandling, bara förvaring. IVO har gjort journalgranskning samt bekräftats av föreståndaren att de inte haft någon ART-träning eller samtal. Två barn som inte är målgrupp har skrivits in. Bland annat en missbrukande, trots att de inte har tillstånd för det. En personal utreds för våld mot en boende.

SVT – Otrygghet, våld och övergrepp, hur kan det förekomma på era boenden?

-  Jag skulle verkligen önska att vi slapp alla typer av avvikelser. Vi kämpar verkligen med ganska svåra barn och ungdomar, och då händer det saker ibland som är svåra att hantera. Det är ytterst beklagligt, och jag är jätteledsen för att det sker och mitt jobb innefattar att inte få det att hända igen.

Skillstreaming HVB, Östhammar

Återigen kritiseras verksamheten för att det inte görs kontroll av registerutdrag av polismyndighetens misstanke- och belastningsregister enligt lag. Samtidigt har verksamheten vidtagit åtgärder för att upprätthålla ett gott bemötande gentemot ungdomarna, men fortfarande finns behov av förbättringar. 

2019

Verksamheten har inte kollat belastningsregistret innan anställning, fortfarande brister med dåligt bemötande mot ungdomarna, personal skriker och en har dragit en flicka i håret ”för att få henne att lugna sig” uppger ungdomarna. Ungdomarna har inga egna nycklar till sina rum, droger har förekommit.

2018

Måste förbättra bemötande, personalen använder sig av begränsande åtgärder och teckenekonomi. Efter tidigare inspektion av IVO har hemmet gett barnen möjlighet att låsa om sig i det ena boendet, men ännu inte i det andra.

Per Drape på Skillstreaming svarar SVT i november:

“Ett dåligt bemötande är självklart inte acceptabelt och bolaget tar kritiken från IVO väldigt seriöst. Bolaget har efter IVOs rapport genomgått stora förändringar med en ny ledningsgrupp och viss personal. För övrigt jobbar bolaget kontinuerligt med att förbättra verksamheten och bemötandet gentemot flickorna.”

Skredsvik HVB, Uddevalla

2020

Återkallelse av tillstånd: Ärendet initierades efter att IVO uppmärksammat allvarlig ekonomisk misskötsamhet hos HVB Väst AB och dess företrädare. IVO har i sin årliga tillsyn också uppmärksammat återkommande allvarliga kvalitets- och säkerhetsbrister.

2019

Våld och stök, flickorna får inte den hjälp de behöver. Outbildad och stressad personal, låg personaltäthet, det är inte föreståndaren som leder verksamheten, bristande kontroll av mediciner. Verksamheten har inte följt IVOs förra krav på förbättringar och de har fler flickor än de har tillstånd till.

2018

Personalen måste bygga förtroende för och samarbete med barnen, skriver Ivo. Måste upphöra med begränsningsåtgärder.

Vassbo Behandlingshem HVB, Uddevalla

2020  

Ett barn med omfattande behov har bott i en stuga där den har varit ensam vissa tider på dygnet, vilket Ivo kritiserar. Hemmet har inte heller gjort lämplighetskontroller skriver Ivo, alltså kontrollerat att ungdomarna som placeras är lämpliga att placera tillsammans. Föreståndaren svarar Ivo att den har gjort lämplighetsbedömningar men inte skrivit ner dem.  

En i personalen har fått sluta efter incidenter med dåligt bemötande mot ungdomar och begränsningsåtgärder. 

2019 

En pojke har utsatts för “fysiska begränsningsåtgärder” och bristande bemötande från personalen enligt en Lex Sarah. 

2018

Ivo kritiserar hemmet för att man brister i läkemedelshanteringen gentemot pojkarna.

“Vi jobbar hela tiden med ett systematiskt kvalitets- och förbättringsarbete och strävar alltid efter att bli bättre på det vi gör. I det ingår att ha en bra samverkan med IVO. I den bästa av världar skulle aldrig några avvikelser uppstå, men vi arbetar med barn och unga med ofta djupt komplex problematik där man kan ha haft ett flertal misslyckade placeringar innan. Vi jobbar med några av de barn och unga som har de kanske tuffaste problemen i svensk ungdomsvård. Och de som jobbar ute på våra enheter gör ett fantastiskt arbete. Men ibland räcker inte ens det. ”

Vision halvvägshus HVB, Östersund

Person misstänks vara påverkad på jobbet. Fick lämna arbetsplatsen. Tidigare missbruksproblematik, Felaktigheter i ungdomarnas personalakter, har inskrivna de ej har tillstånd till, ej anmält sig till vårdgivarregistret trots psykolog på boendet.    

2018   

Ungdomar tar sig under natten in i en annan ungdoms rum där de misshandlar och tar en del ägodelar från rummen. Ungdomarna har tidigare lyckats ta sig in i personalens kontor och stulit nycklar till ungdomens boenderum.   

En placerad ungdom har mottagit textmeddelande, bilder och filmer av sexuell karaktär, skickade av en anställd i företaget.   

Ivo har hösten 2020 beslutat att återkalla tillståndet med omedelbar verkan efter SVT:s granskning.   

Christian Möller är vd på företaget.  

– Det känns åt helvete. Vi har gjort fel men det känns som skitfel, säger han till SVT hösten 2020.

Villa Mea, Malmö

2020

Verksamheten har brustit i trygghet och säkerhet. Kaotiskt, drogpåverkade ungdomar och personal som inte bryr sig, säger ungdomarna.

2019

Två av personalen brister i bemötande, kan inte hantera konflikter och brister i respekt för de inskrivnas integritet. Verksamheten har skrivit in en ungdom som man ej har tillstånd för.

SVT – Otrygghet, våld och övergrepp, hur kan det förekomma på era boenden?

-  Jag skulle verkligen önska att vi slapp alla typer av avvikelser. Vi kämpar verkligen med ganska svåra barn och ungdomar, och då händer det saker ibland som är svåra att hantera. Det är ytterst beklagligt, och jag är jätteledsen för att det sker och mitt jobb innefattar att inte få det att hända igen.

Villa Fridhem, Enköping

Hemmet har använt otillåtna begränsningsåtgärder, inte kontrollerat personalens belastningsregister, begränsat en ungdoms mobilanvändande. Enligt ungdomarna själva har det även förekommit andra begränsningsåtgärder. Droger förekommer ”men mer sällan nu” och en anmälan har kommit in om att en personal agerat ut fysiskt och verbalt mot en ungdom.   

2019

“Tillsynen har visat olika och ibland motstridiga bilder av hur verksamheten på HVB Villa Fridhem fungerar samt om de inskrivna ungdomarna får behandling för sin problematik och om de ges en trygg och säker vård” IVO kan dock konstatera att det har inträffat ett antal allvarliga incidenter.  

2018

Personal höll fast en ungdom och bet honom i örat. Två ungdomar har inte fått behandling visar IVOs tillsyn av tre journaler. Ungdomarna säger att det enda de får göra är städning och ibland öva mindfulness.  

Vid andra inspektionstillfället noterar IVO att bemanningen inte överensstämde med de uppgifter som tidigare lämnats av föreståndaren (två i stället för fyra personal).  

Hemmet gör en Lex Sarah-anmälan då en personal agerat ut fysiskt och verbalt mot en ungdom. Personalen har sagt upp sig.  

Verksamhetens bil blir sönderslagen, det görs inbrott i området och en byggnad tänds på och brinner ner. Polis hittar stöldgods i inskrivna ungdomars rum. Skalskyddet hade inte gått igång. Alla ungdomar utom en skrivs ut från boendet.  

2018  

Ny brand. Hemmet anmäler till Arbetsmiljöverket. “Minibuss som stod parkerad i anslutning till det ena av våra två hus började brinna. Branden spred sig till huset, Utrymdes, inga personskador på vare sig elever eller personal.”
