The provincial accident investigation team determined that the accident was a major production safety liability accident caused by illegal storage and use of civilian explosives and illegal fire operations at the wellhead.
At about 13:13 on January 10, 2021, during the infrastructure construction of the Hushan Gold Mine in Qixia City, Shandong Wucailong Investment Co., Ltd., an explosion occurred in the return air shaft, causing 22 people to be trapped.
After the accident, the leading comrades of the central government attached great importance to it and issued important instructions. The investigation determined that the main cause of the accident was the illegal mixing of detonators, detonating cords and explosives underground, illegal fire operations at wellheads, confusion in the management of civil explosives, and outsourcing management of construction projects, and the lack of responsibility for the implementation of safety production by the involved enterprises. ; Relevant departments are not in place to implement safety supervision responsibilities and local party committees and governments implement territorial management responsibilities.
The investigation also determined that both Shandong Wucailong Investment Co., Ltd. and Qixia City constitute late reporting and concealment.
Based on the investigation of the cause of the accident and the determination of accident responsibility, 45 relevant persons responsible were held accountable in accordance with regulations, disciplines and laws.
To Jia Qiaoyu, the legal representative of Shandong Wucailong Investment Co., Ltd., Wu Zongxing, the project manager of Zhejiang Qifeng Mining Engineering Co., Ltd., who was responsible for blasting operations and mixed explosives with serious violations of regulations, was outsourced to be responsible for the installation of underground equipment and illegal wellhead fires Li Dong, the actual controller of Yantai Xindongsheng Construction and Installation Engineering Co., Ltd. and other 15 relevant persons responsible for the enterprise, were investigated for criminal responsibility in accordance with the law.
A person in charge of the Qixia Project Department of Zhejiang Qifeng Mine Engineering Co., Ltd. died in the accident and was exempt from criminal responsibility.
At the same time, Jia Qiaoyu, the legal representative of Shandong Wucailong Investment Co., Ltd., Yao Xiuxia, then secretary of the Qixia Municipal Party Committee, and Zhu Tao, the deputy secretary of the Municipal Party Committee and Mayor, are currently responsible for the late reporting and concealment of the accident.
28 public officials including the principals of the Yantai Municipal Party Committee and the Municipal Government were given party discipline and government sanctions and organized handling.
