first study published on the outbreak
of monkeypox in the United Kingdom confirms the community transmission of the majority of confirmed cases, in addition to the
prevalence in homosexual environments

gaps in the origin
and causes of its

requires a "concerted international effort".
"The current outbreak indicates a change in basic assumptions about the epidemiology of monkeypox virus with profound implications for surveillance and control," warn the authors of the research published in
Eurosurveillance

Twenty researchers attached to the
national health and safety agency UKHSA
, universities and British public health entities sign the report, which presents as spokesperson Roberto Vivancos, a graduate in Medicine from the University of Oviedo, an honorary professor at the University of Liverpool and an expert in epidemiology.

The investigation remains open and, in this initial phase,
addressed 86 confirmed cases
(one in Scotland and the rest in England) as of May 25.
By June 2, the zoonotic infection had infected a
confirmed total of 225 people in the UK
(214 in England, eight in Scotland, two in Northern Ireland and one in Wales).
Scientists
divide the cases studied into three "signature incidents"
, which raised the alarm on May 7, 12 and 5.
The first did not raise serious concerns: it is an
isolated case imported from Nigeria
, a country endemic to monkeypox, which did not spread the infection among the 116 identified contacts until three weeks later.
In the second,
three members of the same household tested positive or fell ill
, offering no obvious travel links and no source of infection having been discovered.
The third and most worrying incident
began with the detection in England of four infected men
, including a sexual partner, with no ties to the previous cases and no travel connection to endemic countries.
History of AIDS, HIV and other sexually transmitted diseases among those infected were reviewed, although the conclusions of this vein of the investigation have not been published.
"
The data was preliminary or incomplete
", justify the co-authors, who identify other gaps in the knowledge and understanding of the outbreak of the monkeypox virus in Europe.
Among them, they
underline the role of the sexual act in the spread of the current outbreak
and question "whether it can be explained as skin, respiratory or sexual transmission."
Vivancos and his team point to the
difficulty of extracting information in this area.
Of the 82 cases detected in England in the third category, at least 79 are known to be male and the majority (83%) identify as gay or bisexual.
"Several declined to share details of their sexual contacts or reported multiple anonymous contacts, in places such as dark rooms and flirt zones," they explain.
The report also reveals that
18 of this affected group had recently visited non-African countries

In general terms, the
transmission was community
, with an average of four infections per case and up to a maximum of 25, among members of a household, co-workers, friends, sexual partners and health personnel.
Investigators traced links to "
sex establishments, private parties, dating apps, both in the UK and abroad
."
"To date, no single factor or exposure linking the identified cases has been determined," the report concludes.
The research
uncovers a low level of acceptance of the approved smallpox vaccine (Imvanex)
among the medium and high risk population.
Until May 24, it had been administered between 14% of both groups and 69% of health personnel in close contact.
"
Increasing vaccination is essential if the global outbreak cannot be quickly contained
," the co-authors say.
Vivancos and his colleagues support the goal of eradicating the virus, but warn that "success in public health requires a concerted international effort."
