China News Service, Shanghai, May 25 (Reporter Chen Jing) The epidemic prevention and control situation in Shanghai is improving, and medical institutions are gradually returning to the right track.

The reporter learned on the 25th that the Renji Hospital Affiliated to Shanghai Jiaotong University School of Medicine (hereinafter referred to as Renji Hospital) is open to all surgical services, and all surgeries should be received within a limited time period, and an appointment system will be implemented for elective surgeries.

At present, the admission rate of beds in some departments of the hospital has reached nearly 70%, and the daily operation volume of the hospital is nearly 100.

The reporter learned that in order to minimize the risk of epidemic prevention and control in inpatient wards, Renji Hospital has made fine adjustments to the overall layout of the departments of the internal medicine and surgery building, and each building has a buffer floor.

Patients who are admitted to the emergency department but cannot meet the admission conditions will be admitted to a single room on the buffer floor for isolation. Patients who need emergency surgery will undergo personnel protection and negative pressure operating room operations in accordance with the hospital’s hospital infection prevention and control requirements.

Each ward is set up with isolation rooms and a certain proportion of buffer wards.

According to the actual situation, each ward strictly plans "three districts and two passages".

("Three areas" refers to clean areas, polluted areas and semi-polluted areas; "two channels" refers to medical staff channels and patient channels.) The frequency of disinfection has been strengthened in public areas such as wards and elevators. Newly admitted patients can be treated only after terminal sterilization.

Patients admitted to the hospital on different days should avoid arranging the same buffer ward to further reduce the risk of epidemic transmission and ensure the safety of hospitalized patients.

For patients with elective hospitalization needs, Renji Hospital provides full appointment-based services, and the current appointment rate is 100%.

On the basis of meeting the admission conditions, the patients admitted on the same day and their escorts must complete an antigen test. Once abnormal flow patterns, abnormal body temperature or positive patients are found, the medical staff will immediately move them to the isolation ward and arrange for review or transfer. The places and items visited will be closed and sterilized until the environmental sampling is negative.

The hospital explores the establishment of a "building head system", where each head of the building is served by a hospital leader, who comprehensively manages a building.

The "building chief system" ensures that every matter is managed by someone, and every link is supervised, gradually forming a normalized, institutionalized and refined management.

The hospital said that the "building chief system" has "reduced" the risk of epidemic transmission, the movement trajectory of key groups has "stabilized", and the implementation of various systems has "stricken".

According to reports, with the "building chief system", all problems can be resolved on-site by decision-making.

The gate setting rules at the entrance and exit are reasonable adjustments made by the building manager according to the actual situation and after many days of operation observation.

During the closed-loop management of the hospital, each building in the hospital established an emergency leading group.

Led by the head of the building, inspections are carried out on each floor one by one; a classified treatment plan for patients in the hospital is activated, and the directors of clinical departments and head nurses in the building are convened to hold an emergency online meeting to ensure medical safety.

Special staff conducts investigations on inpatients and accompanying family members who stay in the hospital area by area; nurses record the patient's basic information in detail, which is convenient for data statistics and further processing; each doctor evaluates the patient's condition, sorts out the follow-up diagnosis and treatment measures, and patiently informs the patient. and family members to do a good job of explanation and communication, and timely ease the patient's anxiety.

In late April, with the improvement of the epidemic prevention and control situation, the demand for medical treatment and surgery gradually increased. Renji Hospital overcame many difficulties and decided to resume day surgery.

The medical department convened the nursing department, hospital infection office, day management center, day operating room, anesthesiology department, clinical department and other departments overnight to discuss the new process of day surgery under the epidemic.

From the principle of admission and treatment of patients, to the elevator that the patient takes to reach the day ward, every detail is taken care of.

The head nurses in the day ward actively cooperated and refined and reshaped the admission and treatment process for day surgery patients.

It is understood that Renji Hospital is one of the few hospitals in China that conducts daytime minimally invasive cardiac interventional surgery and leaves the hospital on the same day.

Through day surgery, patients can enter and exit quickly, maximizing the turnover rate of hospital beds, freeing up hospital beds and providing them to patients in urgent need of treatment.

Since the closed-loop restart of the general ward of the Department of Cardiology, 102 daytime minimally invasive cardiac interventional operations have been performed from May 1 to May 20 alone, including 85 coronary interventions and 17 pacing electrophysiology.

The hospital told reporters that even when the epidemic prevention and control was the most urgent, the hospital's severe cardiac team continued to treat nearly 100 patients with acute myocardial infarction. The youngest was 31 years old, and the oldest was 90 years old. Cases of acute myocardial infarction; pacemaker treatment of 12 patients with cardiac arrest and Aspen syndrome; including 11 confirmed positive cardiac critically ill patients.

On the same day, in the outpatient building of Shanghai Children's Medical Center (Children's Center) affiliated to Shanghai Jiaotong University School of Medicine, the clinics were in good order.

Tian Dan, director of the Children's Center Office, told reporters that with the gradual opening of society, more and more parents can take their children out of the community and go to the Children's Center to solve medical needs.

At the same time, the hospital coordinated all resources, especially the directors and experts of the clinical departments, devoted themselves to the resumption of work and production.

According to Liu Hong, director of the Department of Ophthalmology, after more than two months of online classes, children's vision problems have become a "heart disease" for parents. Spring and summer are the seasons of high incidence of allergic eye diseases, and the demand for ophthalmology diagnosis and treatment has remained high.

However, due to epidemic prevention and control, most families choose Internet hospitals to solve problems such as eye infections and allergies.

With the gradual opening of the community, the ophthalmology clinic has become lively in the past few days, and she leads the team to wait in the clinic.

"In the days when the lockdown was the strictest, there was not a single patient... Now the situation is much better, the children can see the doctor, and I am most worried about the patients after the operation. I hope they can come to the hospital for follow-up consultation as soon as possible. Many of them have exceeded the review time." Shen Jie, the chief physician of the Cardiology Department, said that they will stick to the outpatient clinic and be able to help the children when they need it.

It is reported that at present, all departments and outpatient clinics of Shanghai Children's Medical Center are open, led by the director of the department and others, and are gradually working at full capacity to meet the medical needs brought about by the gradual opening of the society and the upcoming summer peak.
