Whether it is a county seat or a village, in Zhiduo County, Yushu Tibetan Autonomous Prefecture, Qinghai, many herdsmen can tell a story about themselves and the medical service team of the County People's Hospital.

Zhiduo County is known as "the first county on the Yangtze River", with an average altitude of about 4,500 meters and a population of nearly 50,000. The Tibetan population accounts for about 98% of the total population.

The nearest township is about 20 kilometers from the county seat, and the farthest is more than 200 kilometers. Animal husbandry is the basic industry for herdsmen to survive.

The pastoral area is sparsely populated, and the herdsmen live scattered.

"Considering that some patients are inconvenient to move, take a long time to seek medical treatment, and live alone, in addition, let discharged patients receive continuous health care, and set up a medical service team to carry out 'delivery to home' services throughout the year." People of Zhiduo County Jia Ying, director of the hospital, said.

This team also includes medical experts from Qinghai Provincial Women's and Children's Hospital, so that high-quality medical resources can go to the grassroots level.

During the conversation, Jia Ying and the medical staff packed up medical equipment and medicines and drove off.

About 20 minutes later, we arrived at Jingkang New Village in Zhiduo County.

"Let's see if the patient's condition is better, and brought some medicine..." Jia Ying spoke with the head of the household, Luosang, in Tibetan to ask about his condition, and did not forget to spread some knowledge about disease prevention and health care.

"If you didn't get home, I don't even know what to do." Lausanne said thanks and gave a thumbs up. After the medical staff went through the diagnosis and treatment, they confirmed that the body was fine, and gave medicines, and instructed the usage and dosage.

Lausanne’s wife was sick and had difficulty moving, so the medical staff returned visits from time to time to follow up on the condition, and the current situation has gradually improved, “I used to live in Zhahe Township, and it was not easy to see a doctor in the county town. To see a doctor, I also joined the hospital communication group, and the medical staff will reply to the medical consultation online.”

Continued care is a new model proposed by the County People's Hospital in 2021, which can effectively avoid the disconnection of patients from hospital to family health care. The care objects include internal medicine, gynecology and pediatrics patients.

During this period, the medical staff will also bring the concept of health to the residents, help them change their hygiene habits, and no longer taboo the disease.

"Establish a continuous nursing service center and exchange group to provide offline and online guidance on changes in the condition, precautions for prognosis, and medication. The return visit of different days is also to promote the recovery of patients and reduce the phenomenon of rehospitalization due to deterioration of the condition. If you find that your condition worsens, you can be admitted to the hospital for treatment in time." Jia Ying said that now, the team members clearly remember which elderly people have chronic diseases and which patients have finished taking their medicines.

Since the establishment of the medical service team, medical services have been extended to all villages and towns in Zhiduo.

Among them, Suojia Township, which is the farthest from the county seat, takes nearly 4 hours by car.

One winter, the medical service team went to Soo’s hometown for a free clinic. Because there were no herdsmen along the road, and it was getting late, the medical staff could only sleep in their cars, digging ice and fetching water.

"The medical conditions in rural areas are limited. We will first conduct remote consultations and then arrange referrals according to the situation." Jia Ying said that once he went to the countryside, a Tibetan boy sent Hada to the medical staff to thank the medical staff. carry.

Today, more than 1,500 discharged patients across the county enjoy extended care services.

Ding, ding, ding... At this time, the medical staff's mobile phone popped up the consultation information of the patient's family. After the reply was completed, they prepared the ointment for the treatment of hand joint gout and set off to the next household.
