Spain already has its roadmap ready to turn the page against Covid.

The Public Health Commission has already configured the

key steps for this new transition stage

through the

Surveillance and Control Strategy against Covid-19

.

Starting next Monday, March 28, it will be applied

as long as the indicators for the use of healthcare services are at a low risk level.

Our country, according to the latest data bulletin from the Ministry of Health, has already lowered the pressure in hospitals and ICUs to this range: with 3.78% of beds occupied by Covid and 6.06% in critical care units.

After more than two years of an acute pandemic, the idea of ​​Health and the Autonomous Communities is to take a step towards a transition that represents a turning point.

The

leitmotif

of this new stage is "

a strategy that monitors and directs actions to people and areas of greater vulnerability

and monitors serious cases of Covid-19 and in vulnerable areas and people", as stated in a document prior to approval on the strategy to which EL MUNDO has had access.

In many respects, "

moving to a phase primarily focused on managing severe cases and vulnerable settings and groups is a risk management decision

, based primarily on assessing what levels of residual risk are acceptable to society, weighing the social impact of Covid-19 control measures and the observed decrease in the severity of Covid-19 disease."

For this reason, Health and the Autonomous Communities have agreed to modify the indication for carrying out diagnostic tests, which

will focus on people with vulnerability factors

(over 60 years of age, immunosuppressed and pregnant),

vulnerable areas (health and socio-health) and serious cases

.

Taking this step does not mean that you cannot go back.

The new strategy

will remain in force as long as there is no significant change in the trend that indicates an uncontrolled circulation of SARS-CoV-2

or a change in the epidemiological situation that requires reestablishing surveillance and control measures as proposed by the Conference of Alerts and Preparedness and Response Plans.

Changes in risk indicators

Along with the news that EL MUNDO announced last week related to the elimination of quarantines in mild and asymptomatic patients except for clinical recommendation and the limitation of PCR tests to medical prescription, new indicators are addressed to assess the risk.

In the document to which this newspaper has had access,

the suppression of three of the five indicators of block I is mentioned,

the one that outlines the

transmission and circulation of the virus

.

That is, the cumulative incidence at 14 days and seven days and the positivity of the tests performed are eliminated.

AI is only maintained at 7 and 14 days in patients over 60 years of age.

Currently these indicators still remain high.

The cumulative incidence is still at high risk with 436 cases per 100,000 inhabitants in 14 days.

However, as hospital care pressure and positivity have decreased, the level of alert resulting from the application of the criteria of the coordinated actions document places all communities at level 2 or medium, except Andalusia and Madrid, which are in the level 1 or low.

On the other hand,

in block II, the one that measures healthcare pressure

, two new indicators are added, in addition to measuring the occupancy of hospitalization and ICU beds for Covid-19 cases and the rate of new hospitalizations and ICU admissions for Covid. , per 100,000 inhabitants in 7 days.

From the moment this strategy is implemented,

the level of alert will be established with the indicators of block II or monitoring of healthcare pressure

.

It will be the hospital capacity that determines the greater or lesser impact of the virus on the health system and on society, as the document points out.

To monitor

the transmission of the rest of the age groups

, although in principle the focus is on how the virus behaves in the most vulnerable people, such as those over 60 years of age, residents and people admitted to hospitals,

they will also rely on the System of respiratory infection surveillance (SiVIRA) of the Carlos III Health Institute,

with the detection of cases through the sentinel network that makes up this system.

For this reason, the extraordinary surveillance and control structures put in place during the pandemic will remain operational to monitor key indicators that allow early detection of changes that require the re-implementation of control measures.

Quarantines and limited testing

Mass

screening and quarantine of mild patients become part of the acute stage of the pandemic

Now, the diagnostic effort of the transition phase is focused on the people and areas of greatest vulnerability and serious cases of Covid-19.

As the strategy points out, "the diagnosis of patients with mild symptoms compatible with COVID-19 will be carried out according to the needs of its clinical management. In patients with mild symptoms compatible with COVID-19 and their close contacts, strict isolation and quarantine are not proposed. , and these are replaced by recommendations for preventive measures (use of a mask, avoid contact with vulnerable people...). In vulnerable areas, such as healthcare or socio-health centers, specific isolation and control measures may be implemented".

In addition, it is no longer considered necessary in general "to carry out screening prior to admission or to certain care actions. In any case, hospital centers may consider the use of screening in specific situations and with the aim of protecting patients vulnerable to discretion of those responsible for infection control.
