After investigation, it was determined that the accident was a major production safety liability accident.
The investigation report recommends that seven principals including Luo Rusheng, chairman of Shanghai Longjing Environmental Protection Technology Engineering Co., Ltd., be fined; Zhao Shitong, chairman of Anhui Huasu Co., Ltd., should be admonished; Guo Jiping gave a serious warning to the party, and ordered Anhui Huasu Co., Ltd. to give Li Zhaojun, the director of the thermal power plant of Anhui Huasu Co., Ltd., a penalty of dismissal.
The clues and related materials on the performance of duties of public officials of the local government and relevant departments discovered during the accident investigation have been handed over to the discipline inspection and supervision organs, and relevant units and personnel will be held accountable in accordance with laws and regulations.
Due to their deaths in the accident, Huang Zhong, manager of the Huasu project department of Shanghai Longjing Environmental Protection Technology Engineering Co., Ltd., and Shan Xinlong, a staff member of the project department, were exempted from criminal responsibility.
At about 9:47 on April 7, 2021, a flash explosion occurred in the self-provided thermal power plant of Anhui Huasu Co., Ltd. (hereinafter referred to as "Huasu Thermal Power Plant") in Luqiao Town, Dingyuan County, Chuzhou City, resulting in 6 deaths and direct The economic loss was 9.3537 million yuan.
The top of the slurry storage tank at the accident site was blasted to the tank wall on the east side of the tank body and the pipeline beside the wall of the slurry circulation pump room. The tank top device, maintenance tools, safety facilities, etc. were all damaged in the explosion, and the wall and windows of the pump room were damaged. The insulation material on the side of the emergency slurry tank close to the slurry storage tank was damaged, and the pipeline between the tank farm and the pump room was damaged.
The investigation team determined that the direct cause of the accident was: after the return line of the calcium carbide slag slurry pump in the desulfurization and pulping area leaked, the operators performed welding operations on the top of the slurry storage tank, and the acetylene and carbon monoxide accumulated in the slurry storage tank for a long time. The mixed gas will flash and explode when encountering metal sparks generated during welding operations.
In addition, the accident investigation report found that: Shanghai Longjing Environmental Protection Technology Engineering Co., Ltd. oversighted and neglected the safety production work of the company's Huasu project department.
The project department employs labor in violation of regulations for a long time. There are a total of 54 employees in the project department, of which 46 are labor dispatch personnel, and key positions such as safety specialists and maintenance managers in the project department are labor dispatch personnel.
Anhui Huasu Co., Ltd. did not earnestly implement the main responsibility for safety production, did not have enough understanding of the risks and safety technical characteristics of the calcium carbide slag desulfurization process and equipment, and there were omissions in the identification of risk factors; the company's hot work approval management was a mere formality; outsourcing The safety production work of the unit Shanghai Longjing Environmental Protection Technology Engineering Co., Ltd. has not been effectively coordinated and managed.
The Dingyuan County People's Government has failed to guide and supervise the relevant departments in the jurisdiction and the Salt Chemical Industrial Park in performing their safety supervision duties.
The Dingyuan County Development and Reform Commission did not implement the "Notice on Further Doing a Good Job in the County's Energy Industry Safety Work" issued by the Dingyuan County Safety Committee Office, and did not earnestly learn from the continuous occurrence of safety production accidents in the power industry within its jurisdiction. The large-scale investigation and rectification of the safety hazards of the county's thermal power enterprises were not comprehensive.
The Dingyuan County Salt Chemical Industrial Park Management Committee failed to seriously implement the territorial safety supervision responsibilities, and failed to effectively urge the relevant functional departments to strengthen daily supervision; the safety supervision was not in place, the responsibilities of leading cadres were unclear, the implementation of supervision measures was not effective, and the performance of duties was not in place.
The Dingyuan County Emergency Management Bureau failed to effectively urge the County Development and Reform Commission to implement the special supervision and inspection work on safety production in the power industry, and failed to supervise the implementation of the safety production supervision responsibilities of the Salt Chemical Industrial Park Emergency Management Center.
