It was discovered that a woman in her 60s living in Fukuyama City, Hiroshima Prefecture, was mistakenly vaccinated with the new coronavirus four times.

Fukuyama City wants to thoroughly check the inoculation record and prevent recurrence.

According to Fukuyama City, in June and July, a woman in her 60s who lived in the city received two vaccinations with the new corona virus at a medical institution, and then two more at another medical institution last month. It means that he was inoculated.



Last month, at the time of the fourth vaccination, the medical institution confirmed the vaccination record and found out.



After receiving two vaccinations, the woman contacted the city's call center and reported that she had "lost her vaccination ticket" and was receiving third and fourth vaccinations using the reissued vaccination ticket.

According to Fukuyama City, the woman said she didn't remember how many times she had been vaccinated, and so far she hasn't changed her physical condition.



When Fukuyama City reissued the vaccination ticket, it only confirmed the name and address, but did not confirm the person's vaccination record.

Masamichi Hosoi, chief of the Fukuyama City Health and Prevention Division, said, "In the future, I would like to thoroughly check the inoculation history and reissue the inoculation ticket to prevent recurrence."
