The first in the world!

Father's pancreas + mother's kidneys and kidney failure, son gets "regeneration"

Dad visits son

On August 6, the Organ Transplant Center of Sichuan Provincial People's Hospital completed a special combined organ transplant operation.

The patient is a 27-year-old young man. His parents donated half of the pancreas and one kidney respectively, and saved him, who was struggling with life and death.

The patient, Xiao Miu, has been suffering from type 1 diabetes for many years and has gradually progressed to diabetic nephropathy. Renal failure has occurred and he needs hemodialysis, accompanied by diabetic retinopathy, which is very likely to cause blindness, and the left ear caused by damage to peripheral nerves. Deaf...Xiao Mi's young life, walking on the wobbly tightrope, will fall if he is not careful.

Until August 6th, the ABO pancreas-kidney joint transplantation from the living donor of the two relatives was successfully completed, and Xiao Mi's life was brought back.

According to Yang Hongji, director of the Organ Transplant Center of Sichuan Provincial People's Hospital, this is my country's first relative pancreas-kidney transplantation, the first domestic living relative pancreas transplantation, and the world's first cross-blood type, dual-donor relative pancreas-kidney transplantation.

The 5-year survival rate is less than 20%, a 27-year-old man becomes a patient with end-stage renal disease

According to Hou Yifu, doctor and attending physician of the Organ Transplantation Center, the patient Xiao Miu is a 27-year-old young man with a history of type 1 diabetes for more than 10 years. He has been relying on insulin therapy to control his condition, but he still developed diabetic nephropathy.

More than a year ago, Xiao Miu's creatinine began to rise, which shows that his kidney function has been damaged.

At the same time, vision began to decline and developed into "diabetic retinopathy."

More than 3 months ago, Xiao Miu's condition became more and more serious, more target organ damage, deafness of the left ear, renal failure, and he had to undergo dialysis treatment.

"In the case of insulin, the fasting blood glucose level of the patient is still as high as 20." Yang Hongji, director of the Organ Transplant Center of Sichuan Provincial People's Hospital, explained that this is very dangerous.

According to reports, the 5-year survival rate of patients with end-stage renal disease is less than 20%.

Organ transplantation is the best option

Parents become donors, but cross blood type is a big difficulty

Organ transplantation has become the best plan to save Xiao Miu's life.

But if you wait for the organ donation, there is a shortage of donors and a long waiting time, Xiao Miu may not be able to wait for the day of the transplant.

“Kidney transplantation between relatives is already a mature technology, but there is no report on living-donor pancreas transplantation between relatives at present, and no transplantation centers across the country have done pancreas live donation.” Yang Hongji said, the pancreas donated by citizens after their deaths are A whole section of the pancreas is used for transplantation, but only a part of it can be used for living organ transplantation between relatives, which requires very high donor quality and transplantation technology.

At the same time, organ transplantation of relatives across blood types is also a difficult point.

"The father's blood type is the same as the son's blood type, it is type A, and the mother's blood type is type AB." Yang Hongji explained that in principle, blood type AB cannot be a donor of blood type A because it will produce type B antibodies, which will severely attack the recipient's organs.

But with the exploration in recent years, it has been possible to overcome this obstacle through the elimination of type B antibodies.

Based on the successful experience of cross-blood kidney transplantation, doctors chose 49-year-old mothers of different blood types as the kidney donors when formulating the surgical plan, and 51-year-old fathers of the same blood type as the pancreas transplant donors.

  The world's first case, success!

Parents' organ transplants extend son's life

At 8 o'clock in the morning on August 6, a 12-hour combined organ transplant operation began.

The first person who was pushed into the operating room was Xiao Mi's mother. The doctor wanted to remove her left kidney.

Later, Xiao Mi and his father underwent operations in two operating rooms at the same time.

The mother's left kidney was transplanted into Xiao Mi's left iliac fossa, and my father's pancreas was successfully obtained and successfully transplanted into Xiao Mi's right iliac fossa.

"The length of the blood vessel is not enough to intercept the body and tail of the father's pancreas. We also need to reconstruct the arteriovenous blood vessels." Yang Hongji said.

The operation, which lasted until 8 o'clock in the evening, was a success!

"The (transplant) effect is very obvious. During the operation, the patient's urine output exceeded the normal urine output value." Yang Hongji said that the patient's blood sugar returned to normal on the first day after the operation.

On the third day after the operation, creatinine also returned to normal. This is an excellent news-it proves that part of the pancreas donated by a living body can completely correct the diabetic disease state of diabetic nephropathy.

This means that Xiao Mi can get rid of insulin or medication after surgery, no longer need to undergo dialysis, and can completely resume normal social activities and health.

The other good news is that the donor’s parents also recovered quickly after the operation.

At present, the patient's mother has been discharged from the hospital and returned to her hometown, and the father's blood sugar level has returned to normal.

Yang Hongji introduced that this is my country's first relative pancreas-kidney transplantation, the first domestic living relative pancreas transplantation, and the world's first cross-blood type, dual-donor relative pancreas-kidney transplantation.

Xiao Miu’s mother, Ms. Hu, who has returned to her hometown to recuperate, told reporters that her son discovered diabetes at the age of 11.

Over the years, the child’s illness has caused the family to suffer.

In February of this year, because his son became seriously ill again, he went to the provincial hospital to see him.

Knowing that organ transplantation can save the child, Ms. Hu said that she and her husband both took the initiative to ask for donation, "For the child, we are willing to do everything, and now it is finally better." Ms. Hu said that after the operation, her body recovered quite well. , The child's father is also very good.
