Nucleic acid samplers in Shunyi City sampled at

minus 10°C, completing more than 400,000 cases in one month

Take the swab, reach into the throat to sample, put it into a test tube, disinfect your hands, and wait for the next one.

The doctors and nurses of the community health service center (hereinafter referred to as the health center) of the Shunyi District Urban Community Health Service Center (hereinafter referred to as the health center) have experienced more than 400,000 times in the past month of these throat swab sampling procedures.

On December 25 last year, a confirmed case of new coronary pneumonia appeared in Shunyi District, and then a nucleic acid test for all employees was carried out.

On January 10 this year, Shunyi District added 1 more confirmed case and 4 cases of asymptomatic infection to carry out the second full-time nucleic acid test.

Shunyi City is one of the most densely populated areas in the entire district.

In the two rounds of full-staff testing, the health center completed sampling of nearly 180,000 people in the urban area.

"We haven't taken a break this month." Zhao Lihua, director of the Health Center, spoke with a hoarse voice. Working with the neighborhood committee made her throat exhausted, but the spirit in the words was still not halved.

"Put on more clothes to guard against the security department"

At around 5 a.m. on December 25, 2020, Tian Jing, who was still asleep, received a call from Zhao Lihua: Put on more clothes to prevent the security department from gathering.

Although she didn't explain the task, Tian Jing had already guessed it, "Is it going to sample again."

Tian Jing is a nurse in a health center.

When the Beijing epidemic broke out in June last year, she and her colleagues sampled nucleic acids from key groups such as express delivery and supermarkets in Shunyi City. Several hundred people were sampled every day, which was considered a "veteran."

The task this time is to sample a suspected case in Building A, Yibin South District, Shengli Street. All residents of the entire building should be sampled for nucleic acid.

After arriving at the scene, she discovered that Building A had been closed.

A sentry box downstairs was sprayed and disinfected with 84 disinfectant in advance.

Four packs of encapsulated protective materials are placed inside, and each bag contains protective clothing, hats, masks, shoe covers, etc.

The guard box was closed, waiting for Tian Jing and other colleagues to enter the dressing.

That morning, Tian Jing started working without breakfast.

She told the Beijing News reporter that when the epidemic broke out in the Xinfadi market last summer, if you eat and drink before working, you may waste a piece of precious protective clothing for going to the toilet.

"I don't know if the protective suit is short (missing) this time, so I didn't have breakfast."

The lowest temperature in Beijing this day was minus 4 degrees Celsius.

Zhao Lihua repeatedly urged Tian Jing to wear more, and sent a new down jacket to the nurses alone, and Tian Jing put two down jackets in the protective clothing.

Before deciding to do so, she had a brief period of entanglement: for a while, should she sample in the house or collect the sample downstairs?

If samples are taken in the house, steam may be generated in the protective clothing because of the heat, which will cover the goggles.

"Prefer to be slow, not to be wrong"

Community personnel set up 4 tables downstairs to disinfect the environment around the courtyard.

After the sampler is seated, the test begins.

Everyone in the building came out to do nucleic acid work.

In order to avoid possible cross-infection, the organization method of community members is to "go back to one household and leave one."

Every time a household is tested, the site is disinfected to prevent aerosol infection.

Everyone sampled in this building may have been exposed to a positive case.

The head of the Department of Prevention and Protection urged Tian Jing, “It's better to be slow than to make mistakes.” In order to make the test results accurate, Tian Jing probed the swab deeply and wiped the pharyngeal palate on both sides of the back wall of the pharynx in accordance with the operating specifications learned during the training. Bow, "You must pick it on both sides."

  Not every sampler works together.

At the table next to Tian Jing, a nurse felt that the first sampling was not in place and wanted to do it again.

The other party was an elderly uncle, and he caught the little nurse and scolded him.

Fortunately, in the end, the uncle took another sample.

Detection has been done at 1 o'clock in the afternoon from the morning.

Finally, Tian Jing and another nurse went to the home of the positive case to sample the people living with him and the internal environment.

They first sampled the iron door of the unit building and the stair railing outside the door of the household, and then entered the home to continue sampling the toilet, kitchen, used computer, and remote control of the case.

The sampling site is the place where the object touches the hand, such as the grip of the remote control. Follow the zigzag gesture to wipe the sample.

Tian Jing remembered that she had collected more than 20 environmental samples in total.

After December 25, Tian Jing also visited Building A, which was closed three times, sampling all the staff each time.

She did not remember the face of a certain resident, but noted that the number of people tested in the entire building was 204.

  A tough battle has started

On the evening of the 25th, Zhao Lihua, director of the Health Center, received a notification of nucleic acid testing for all employees in Shunyi District.

Her task is to complete the sampling work of 180,000 permanent residents in the three streets of Shengli, Guangming and Shiyuan in the city starting on the 26th.

The preparation time left for her was only a dozen hours.

On the evening of the 25th, Zhao Lihua and seven or eight colleagues distributed protective supplies to the doctors and nurses, one for each person.

The bag contains protective clothing, masks, gloves, goggles, alcohol spray, etc.

Careful Zhao Lihua found that some swabs had to be pushed out from the middle before use, but they were too hard, so they put small scissors into the material bag.

The advantage of distributing protective materials is to avoid the chaotic scene of taking materials.

Outside the health center, 54 collection points for urban neighborhood committees are also under construction.

On the evening of the 25th, Zhao Lihua and his colleagues rushed to several large-scale collection sites with thousands of samplers to ensure that the layout and process meet the hospital-sensing standards.

"Which way to go in, how to register, how to line up, and then after the collection, how to get out, can't cross." It was 3 am after everything was set up.

On the morning of the 26th, a tough battle started.

Early in the morning, all medical staff were on duty.

There are 54 spots in the jurisdiction. Zhao Lihua's vision is to arrange 4 to 6 people per spot on average, and 2 to 4 people in small spots.

Zhao Lihua hopes that three shifts of rotation will be arranged at each point, so that the doctors and nurses can get enough rest.

She quickly settled the accounts for the reporter: Assume that there are 20 spots on a street, and an average of 2 people at each spot at the same time, a total of 40 people in a shift, and 120 people in a third shift.

However, the health center can only dispatch about 100 people.

This is based on the fact that the health center and the 15 subordinate service stations mobilized all the staff, and only the staff on duty in the pharmacy and 2 doctors were reserved.

Therefore, manpower shortage is inevitable.

That morning, almost all personnel from the health center were dispatched.

She needs to "call for reinforcements" before the shift in the afternoon.

Fortunately, she was supported by the Shunyi District Health Commission and the Beijing Municipal Health Commission.

Reinforcement troops from other hospitals and health centers arrived soon. In the end, a total of three to four hundred doctors and nurses joined the general mining in Shunyi City.

"People helped us, we can't help others"

On the first day of Pucai, Tian Jing worked in a square in the Yibin Street community.

A director of the neighborhood committee told her that there were more than 5,000 people sampled at this point.

General mining uses a ratio of 1:10, and puts a test tube for every sample of ten people.

Two tents were set up on the spot, the inspection channel was separated by isolation tape, and a meter line was drawn on the ground.

As the sampling ratio has increased, Tian Jing must hold the test tube in her left hand and take the swab handed over by her partner in her right hand for sampling.

She could not put down the test tube until the 10 swabs were filled in a test tube to prevent the preservation solution in the test tube from pouring out or the swabs to touch the table.

In a few days, Tian Jing has handled at least thousands of samplings without any mistake.

Cold hands are the biggest distress of frontline samplers.

At the end of December, Zhang Meijuan, a medical staff member of the Health Center, performed nucleic acid sampling at Hualian Shopping Center.

The lowest temperature that day had dropped to minus 6°C, and no shelter was erected at the sampling site.

At the end of each sampling, the partner must spray alcohol disinfectant on her hands, and she needs to wait for the alcohol to dry before taking the next sample.

Across two layers of rubber gloves, the heat continued to radiate from her hands, "just like an ice cellar." After work, her and colleagues' hands were swollen and swollen, and they hurt for several days.

On January 10 this year, Shunyi District added 1 more confirmed case and 4 cases of asymptomatic infection. Shunyi District subsequently announced that it would continue the second full-staff nucleic acid test.

Zhao Lihua sent Tian Jing and other 10 medical staff to support him, "People have helped us, and we have to help others."

At the moment, the Health Center is conducting concentrated supplementary tests on the missed and missed personnel identified by the neighborhood committees, and nucleic acid tests on relevant personnel in Hebei, entry personnel, and personnel who have been isolated and released from close contact.

In a work group where Zhao Lihua is located, the person in charge of each community health center in Shunyi reports up the number of samples tested that day.

The numbers reported by urban health centers are often the most, ranging from two to three hundred to thousands.

During the interview, Zhao Lihua received another call from two neighborhood committees and found out that there were residents who missed the test.

"When is this, why is it still not tested?" After a few words of anger, Zhao Lihua went to work again.
