On December 31, 2020, at the Shunyi branch of the Beijing Emergency Center, staff killed the ambulance.

Nine days have passed since Beijing notified residents of a community in Shunyi on December 24 last year that they were confirmed as asymptomatic.

During the 9-day period, how did the close contact personnel be transferred, and how to ensure protection and daily life when transferred to the isolation point?

Recently, reporters from the Beijing News visited the Shunyi branch of the Beijing Emergency Center and a centralized isolation and observation hotel in Shunyi for on-site visits.

Location: Shunyi 120 Emergency Center

Depart within 2 minutes after receiving notification from the close contact

During this round of the epidemic, the Shunyi 120 Emergency Center was not only responsible for the transfer of confirmed and suspected cases in the entire district, but also for transferring close contacts to designated isolation points.

From the night of December 23 to the early morning of December 31, 2020, a total of 1,551 transfer tasks were completed here in 8 days.

Peak transit time is concentrated at night

The reporter saw from the pre-hospital medical record that at 0:00 on December 31, 2020, the emergency center received a call from the disease control department, confirming a close contact of a new coronary pneumonia case.

The call comes at 0:01, the order is received at 0:01, and the ambulance departs at 0:02.

With long-term first aid experience, the staff has already developed the ability to act quickly within 2 minutes after receiving instructions.

That day and night, the Shunyi branch center completed the emergency transfer of more than 300 people who were closely connected with new coronary pneumonia cases.

Zhang Shuxin, director of Shunyi 120 Emergency Center, told reporters that since the 23rd, their peak work hours have been concentrated at night.

Each vehicle is equipped with doctors, nurses, and drivers, and the three are in groups.

Kan Xiufeng, deputy director of the emergency department outside the hospital, followed 7 trains this night. The temperature has dropped to minus 10°C. However, due to the size of the protective clothing and the convenience of work, he can only put on two more warm underwear each time. Layer isolation protective clothing.

"Although it is not warm, but at least wind-resistant."

At 8 o'clock in the morning, Kan Xiufeng finished his duty, took off his mask, hat, two layers of gloves and shoe covers, took off his protective clothing and isolation clothing, and entered the duty room to put on his down jacket after disinfection.

The fingers were frozen.

31 emergency vehicles in the city support Shunyi

According to Zhang Shuxin, the emergency vehicles for transporting people involved in the epidemic were separately deployed and arranged by the hospital.

The Shunyi District Hospital has arranged 15 ambulances, which are responsible for the transfer of close contacts, fever, suspected or confirmed patients.

At the same time, the Beijing 120 emergency center and the suburban emergency centers also sent a total of 31 ambulances and medical personnel to support.

Emergency center personnel are the first people to come into contact with confirmed or close contacts, and their protection work is extremely important.

Zhang Shuxin told reporters that, in addition to being fully armed, he immediately issued masks, hats, gloves, shoe covers and other disposable protective equipment to the parties after arriving at the homes of relevant personnel. During the whole process, no contact with staff.

After completing the transfer task, the staff will immediately place the spray disinfection equipment in the car, adjust the nozzle, turn on the power, and close the door.

After 5 minutes, take out the equipment, then close the door and wait for 40 minutes, airtight and disinfect.

Finally, open the doors and windows for ventilation, and the disinfection work is completed.

According to Zhang Shuxin, the staff responsible for killing may repeat hundreds of times a day at most.

To Zhang Shuxin's satisfaction, since the transfer of the first confirmed case, none of the team members has been infected.

According to Zhang Shuxin, in addition to the staff in the 120 undergraduate room, the district hospital also dispatched 55 medical personnel to support this work.

The staff of the liaison group told reporters that sometimes when they encounter close contact persons who are nervous and scared, they all explain patiently.

Many people's voices became dumb because they kept talking and explaining for seven or eight hours.

Location: Quarantine Hotel

Shunyi 14 hotels to undertake close contacts

At present, Shunyi District has arranged a total of 14 hotels to undertake the centralized isolation medical observation needs of close and sub-closers.

The reporter came to a centralized isolation observation point currently in use in Shunyi District. The hotel is a three-building five-story structure that can meet the isolation observation needs of about 300 people.

Strict distinction between occupants and medical channels

The person in charge of the isolation point, Xiao Xue, the first-level director of the Shunyi District Health Committee, said that in order to ensure the safety of epidemic prevention, the hotel has divided the passage of people into, the passage of isolation and the passage of medical staff.

In addition, partitions were added to the junctions between the three buildings to achieve physical isolation, and all stairs were sealed off.

According to the requirement of "one vehicle, one person", the close contact was sent to the isolation point by 120 ambulance and entered through the back door of the hotel.

The first thing to do when entering the hotel is to take a temperature measurement.

After that, in addition to daily body temperature testing, nucleic acid testing is required on the first 1, 3, 7, and 14 days as required.

Once it is positive, immediately contact the health department and send it to a designated hospital for treatment by a negative pressure ambulance.

Persons who are safely released from quarantine must be brought home by the staff of the territorial department.

It is convenient to ensure that personnel complete the 7-day quarantine observation at home and can be traced after the quarantine.

From the side entrance of the hotel, you can enter the staff office area. The reporter saw that the floor and walls of the entire corridor were completely wrapped by plastic film.

Xiao Xue told reporters that on the one hand, this is to prevent frequent spraying of disinfectant from damaging the floor and walls, and on the other hand, the smooth surface is not conducive to virus attachment.

The doctor goes to the quarantine area for rounds every day

The staff at the isolation point are divided into three groups: the prevention and control disinfection group, the humanistic care group and the logistics support group.

The 21-year-old Liu Tanyu was the first batch of resident doctors in this isolation point.

On December 8, 2020, she was transferred to the hotel from the Feng Bo Health Center to be responsible for receiving people in isolation.

Starting from December 27, 2020, the quarantine hotel will be responsible for cooperating with the close contact personnel's quarantine task, and Liu Tanyu has also become a staff member who has face-to-face contact with the close contact persons.

"Dr. Liu, how do I feel a little fever today." When the quarantine observer feels an abnormality, he will call Liu Tanyu's work cell phone.

On the one hand, it is necessary to solve the treatment of physical symptoms, and on the other hand, it is necessary to promptly relieve the anxiety and tension of the parties.

Liu Tanyu said that there have been several cases of fever caused by emotions, and after treatment and counseling, "the alarm was lifted."

Xiao Xue introduced that the on-site medical staff follow the hospital's work model and go to the isolation observation area to enter the ward every day, meet with the isolation observers, take their body temperature, and observe changes in the other party's mental state.

Medical personnel entering the isolation observation area shall be protected according to the guidelines.

In addition, the medical staff is also responsible for the supervision and guidance of the disinfection work, and instructs the hotel staff to perform disinfectant ratio, disinfection records, and work handover.

It is reported that Shunyi coordinates the hotel resources of the entire district, and prioritizes the expropriation of hotels with many rooms and large volumes to ensure that they can concentrate their efforts to undertake security tasks and avoid distracting the energy of various departments.

Hotel service personnel and special class personnel carry out nucleic acid tests once a week, once a week; isolation and observation personnel strictly implement "single rooms", except for children under 14 years old, seniors over 70 years old, and people with related diseases who need care and pass through the community The group agrees that it is strictly forbidden to live together except for the three types of personnel.

■ Dialogue

Zhang Shuxin: I can hold on, and the staff at the emergency center can hold on

"I'm going to get busy in a while. Today's lunch break is gone." At noon on December 31 last year, the reporter met Zhang Shuxin, the busy director of the Shunyi 120 Emergency Center.

Just the night before, she had been on duty at the emergency center all night, dispatching more than 100 close transfers of personnel.

Beijing News: Compared with the outbreak that just appeared in early 2020, has the emergency transfer work changed now?

Zhang Shuxin: At the beginning of last year, medical supplies were in short supply. In order to save protective clothing, our group of children stood outside in the cold wind after picking up people, waiting for the next pick-up instruction.

Because he had to take off his protective clothing for disinfection when he entered the house.

At the longest, a set of protective clothing lasts for 8 hours.

  The supplies are now sufficient.

However, the temperature has dropped drastically in the past two days, and there is no way to wear a thick down jacket when wearing protective clothing.

Protective clothing does not dissipate heat but does not keep warm. On the night of December 29, 2020, when the body temperature was minus 20°C, our staff went out to pick up more than 300 people.

Beijing News: Why is the emergency center transfer mainly at night?

Zhang Shuxin: Actually helpless, because the flow adjustment work can only be carried out during the day, it was already night when we got the list of close contacts.

Therefore, our working hours have changed from eight or nine in the evening to five or six in the morning the next day.

A maximum of 36 ambulances operate at the same time. This epidemic also taught me a more reasonable and efficient deployment arrangement.

As long as I can stabilize, the staff of the emergency center can stabilize.
