Sister's little feet

Guangzhou Daily News (all-media reporter Weng Shuxian, correspondent Wang Hui, Huang Xiaoxia, Bai Tian, Liang Jianzhong photo report) This year 31-year-old A Ting (pseudonym) gave birth to phoenix fetus at 25 weeks of pregnancy. At birth, her siblings were only 465 grams and 670 grams, only as big as an adult female slap. The neonatology department of the Third Affiliated Hospital of Guangzhou Medical University launched a life-long escort system. Medical staff continued to rescue the baby for 116 consecutive days and helped the baby through several life and death passes. The weight reached 2.35 kg and 3.34 kg, respectively, and was discharged from the hospital on August 11.

According to statistics, since 2006, my country has successfully treated less than 10 cases of ultra-low birth weight infants under 500 grams. In this case, the elder sister weighed only 465 grams, which was the lowest weight premature baby successfully treated by the Third Hospital of Guangzhou Medical University and the lowest weight premature baby successfully treated in Guangzhou.

"It is said that the child fell to the ground, but I did not have the chance to hear the first cry of the child coming into the world." Ating's initial impression of her child was "small and red." The doctor gathered around and rescued. Recalling the scene when she was lying on the delivery bed and glimpsed from the corner of her eyes, A Ting's eyes were red.

When the 31-year-old was pregnant with twins, the family was very happy. However, this happiness did not last long. On February 8th, she went to the Third Hospital of Guangzhou Medical College due to abdominal pain. The examination showed that there was a threat of miscarriage, and she was hospitalized for abortion. However, in early April, Ating still broke the water ahead of schedule. The Obstetrics Department of the Third Hospital of Guangzhou Medical University did its best to continue to protect her. Until April 17, the younger brothers, who were only 25 weeks old, could not wait to come to the world in advance.

At birth, the two babies weighed only 465 grams and 670 grams, both of which were as big as the palm of an adult female. In particular, the "sister" of less than a pound, 465 grams, has an arm not as big as an adult's finger, and a foot is not as big as an adult's thumb. The skin on the whole body is "breakable" by blowing and the blood vessels are clearly visible.

The Department of Neonatology of the Third Hospital of Guangzhou Medical College had a clear understanding of A Ting's situation before the twins were born. The "Special Action Team for Baby Care" headed by Professor Cui Qiliang of the Department of Neonatology formulated a detailed rescue plan.

On April 17, the two elder brothers gave birth 40 minutes apart. They breathed weakly at birth, and their organs were extremely immature. According to regulations, tracheal intubation should be completed within 20 seconds for neonatal treatment. Because the two siblings are too small, the smallest domestic laryngoscope is too large for the two siblings, which brings great challenges to the rescue. Skilled doctors Huang Xiaoxia and Zeng Jianfei completed the tracheal intubation of the newborn in a few seconds and used the T-combination resuscitator to ventilate the newborn with positive pressure. However, the siblings still breathe very weakly and laboriously under positive airway pressure ventilation.

"This is a typical neonatal respiratory distress syndrome." said Wu Fan, chief physician of the neonatology department of the Third Hospital of Guangzhou Medical College. Therefore, within 10 minutes after the baby was born, the rescue team treated them with pulmonary surfactant, and quickly put them in a newborn incubator with suitable temperature and humidity, and transferred them to the NICU in the neonatal intensive care unit.

With a small gestational age and such a light weight, the two brothers and sisters were rescued and transferred to NICU in the first place, but they still faced many life and death challenges.

Premature birth caused the child's various organs to develop stunted. The respiratory and immune systems of the two siblings were extremely fragile. The two siblings encountered another "hurdle" on the way home-severe pneumonia. Because the siblings were too young when they were born, the existing domestic tracheal intubation and ventilator nasal congestion are all oversized for them, and the accuracy of medical equipment is not suitable. This is a big problem in the treatment. To this end, the Department of Neonatology of the Third Hospital of Guangzhou Medical College sent the most experienced medical staff to relay treatment, improved nasal congestion, carefully adjusted the parameters of the ventilator, rechecked blood gas, ensured circulation, resisted infection, and overcome feeding intolerance, anemia, electrolyte disorders... In more than 100 days, the medical staff took turns watching them 24 hours a day, step by step "steadily fighting" and finally helping the brothers and sisters successfully pass the barrier.

"This is the lowest weight premature baby we have successfully treated." Wu Fanxin said with joy.
