Later, he was transferred to Wuhan Pulmonary Hospital together with Hu Weifeng, another doctor who was infected with new coronary pneumonia in Wuhan Central Hospital.

  That is, while Yi Fan became ill, from January 23, the China-Japan Friendship Hospital has sent six batches of 164 medical teams to support Wuhan. As a member of the third batch of medical teams sent by the hospital, Zhan Qingyuan arrived in Wuhan on February 1 to work in the Sino-French New City Campus of Tongji Hospital. The colleagues introduced Yi Fan and Hu Weifeng to Zhan Qingyuan and invited him to consult with the two doctors.

Zhan Qingyuan: We are peers, and they are unfortunately infected with the new crown virus at work. Yu Gong is private and reasonable, and I especially want to rescue them.   On March 2, Hu Weifeng was transferred to the Sino-French New City Campus of Wuhan Tongji Hospital where the medical team of the China-Japan Friendship Hospital was located. On March 3, Dr. Yi Fan transferred in.

  When the two doctors were transferred to the ward in charge of the medical team of the China-Japan Friendship Hospital, they were in a complete coma. The respiratory and organ failure caused by the new coronary pneumonia, together with other complications, the situation of Yi Fan and Hu Weifeng was full of uncertainty.

Zhan Qingyuan: According to the requirements, you can't go wrong in every link. Once there is a problem, the patient may turn sharp and irreversible, and fall like we walked a tightrope. Moreover, the treatment of this disease is not immediate, it is a very long process, but the longer the time, the greater the risk. I have confidence in my team, but I must also have relatively high demands on them, otherwise this battle may be lost in a certain link.

"Two Hearts and Three Lungs" keeps life ready to deal with major bleeding

  Prior to this, the doctor conducted endotracheal intubation with Yi Fan and Hu Weifeng, and had been to ECMO twice. After CT, Zhan Qingyuan judged that the lung recovery of the two doctors would be very slow. In this case, he decided to give them ECMO again, the purpose is to give them enough time to recover, while treating their complications.

  Zhan Qingyuan: ECMO itself is both a personal heart and a personal lung, so it is equivalent to two hearts. Normally, a person has one lung, and if he has a ventilator, it is also counted as one lung, and the artificial lung is three lungs.

  Reporter: The vital signs of the two are completely supported by the hardware to maintain such a stable state?

  Zhan Qingyuan: Yes, without these supports it is difficult to imagine that they can live to that time. In addition to doing CT for them, the second thing was that both patients had tracheotomy. Once a person wakes up, all his physical defense functions can be mobilized, so they need to be cut when they are not awake. However, there is a particularly big risk of incision, because the anticoagulant we give is not easy to coagulate, and it is prone to major bleeding, which is easy to see. We also actively dealt with the problem of bleeding, and finally stopped it.

Have to replace the ECMO catheter "three twists and turns, thrilling"

  On March 5, Dr. Hu Weifeng developed symptoms of infection. Wang Chen, an academician of the Chinese Academy of Engineering and the former president of the Sino-Japanese Hospital, came to the Sino-French New City campus to work with the doctors of the Sino-Japanese Hospital on the consultation of Hu Weifeng's condition. He made a decision to replace the catheter for the medical team.

Wang Chen: I will help you make a decision. You may also find it difficult to change the tube. This decision may not be easy to make, but you ca n’t live without it. I think the problem with the tube must still be changed. At this moment we have no other means.

  Both Yi Fan and Hu Weifeng have blood-borne infections, which means that every effort must be made to remove the source of infection in the blood. The most important and critical thing is to replace all catheters on them, especially the most dangerous ECMO catheters. However, replacing the ECMO catheter was thrilling and twisting.

  Zhan Qingyuan: We failed to change Hu Weifeng for the first time, and a big complication was bleeding. At that time, I said that you should stabilize your blood pressure. At that time, I thought that as long as my blood pressure was maintained, I would go on stage to stop the bleeding, and I would have to go on stage even if I tried hard.

  Zhan Qingyuan: Because he had plunged that place many times, it was indeed very difficult to operate. As soon as the pressure increased after the opening of ECMO, the small mouth became a big mouth, and the blood blew out. We immediately pulled out this tube, or changed it to its original position, and the blood stopped when we were doing ECMO.

  After Hu Weifeng's ECMO pipeline was successfully replaced, Yi Fan's ECMO pipeline was also replaced. This action caused the infection indicators of the two doctors and patients to begin to decline, indicating that the infection was cleared. On March 6, Hu Weifeng regained consciousness. On March 7, Yi Fan woke up.

Before evacuating from ECMO, Yi Fan “deceived” like this

  In the intensive care unit of New Coronary Pneumonia, medical staff often persevered not because they saw hope, but because they did not give up perseverance. After multi-disciplinary medical treatment and guardianship of medical staff, Yi Fan broke away from ECMO's life support on March 15. Before leaving ECMO, Zhan Qingyuan told Yi Fan that his ECMO had been closed for two days. The two days depended entirely on the ventilator, and had nothing to do with ECMO. The offline test was very successful, giving Yi Fan confidence in leaving ECMO.

  Before going offline, Yi Fan wanted to drink water. Zhan Qingyuan fed Yi Fan with a cotton swab dipped in water and told him "This water is sweet dew."

  Zhan Qingyuan: ECMO gave him a more critical step. The ultimate goal was to let him out of the hospital, but it was still not possible to leave the hospital. He had to return to society.

  Zhan Qingyuan: It ’s not important, it ’s the most important, because he puts his life in your hands, whether he himself or his family, or Wuhan Central Hospital trusts us 100%, so I am particularly touched by this, but they are also all Knowing that these two patients is indeed very difficult, I must rescue him to the best of my ability, at all costs.

  Yi Fan's condition began to improve, but Hu Weifeng recovered slowly. One day he even started to develop a high fever. Considering that it was caused by infection, Zhan Qingyuan risked extubating Hu Weifeng.

Zhan Qingyuan : At this time I have to take risks, and I risk taking him out. Because I think he may have a lighter infection after pulling out, his breathing rate may be down, and it may still be maintained by the ventilator. It is not strong for a few days. I will give him another ECMO, although that is the fifth ECMO. But I can interrupt the infection. After the withdrawal, he really couldn't bear it, and the carbon dioxide in the blood reached more than 100 that night. Finally, after carrying it for about seven or eight days, the breathing rate gradually improved.

  On March 22, Dr. Hu Weifeng also successfully withdrew ECMO. On April 6, the day before the evacuation from Wuhan, soldiers in white who fought for more than 70 days at the China-Japan Anti-epidemic Medical Team of Beijing Sino-Japanese Hospital in Wuhan had bid farewell to Yi Fan and Hu Weifeng. Hu Weifeng was very excited and wanted to express his gratitude, but was still unable to speak. He took the pen in his hand and wrote the words "miracle in medical history!".

Zhan Qingyuan : This is a kind of affirmation to us, but also a kind of gratitude, and also a mutual understanding and trust between comrades in arms.

  The farewell video before the evacuation moved many people. The blackness of the two doctors and patients caused great public concern.

Zhan Qingyuan: We also had his early photos. At that time, it was a bit dark. Later, because of the longer time of using polymyxin B, it became darker and darker, but it is not good for other organs and patients. Too much impact. If he recovers later, he can recover slowly.

Hu Weifeng suffered cerebral hemorrhage, "I believe that old Hu can survive"

  On April 7, 164 white-clothes soldiers from the National Aid Medical Team of the China-Japan Friendship Hospital successfully completed the anti-epidemic mission in Wuhan, and won a safe victory. After Zhan Qingyuan returned to Beijing, Dr. Hu Weifeng was transferred to the general ward on April 14. Zhan Qingyuan: I believe that Lao Hu can survive, and Lao Hu is really dead. It is not fatalism to say that our fate is very big. The vitality of his own body and his ability to repair are different from human to human. I think that the vitality of Lao Hu is quite strong, and I still hope for him.

Yi Fan was discharged home: he had n’t breathed oxygen in the past 20 days and tried to return to work soon

  On May 8, Yi Fan was discharged home and returned home. Accompanying her daughter's online class, dancing, and doing rehabilitation training is Yi Fan's daily life. With the gradual improvement of his physical condition, the previously darkened complexion also turned white.
