In the department of mental health medicine at the university hospital where I was training, I held a small award ceremony every year. Is the best clinical professor award. It might be like a professional baseball MVP award ceremony. What is the standard in psychiatry?

You can't vote for fellow players or press because it's not being relayed like an athletic game. You can't get ratings from patients. Healing good words does not necessarily mean good therapy. The answer was eventually. If you are looking for a lot of people who came to see you again is likely a good doctor.

The performance of the MVP candidates was huge. 20,000 people. One psychiatrist has seen more than 20,000 patients see a year. Every night after more than 100 outpatients, the MVP professor, who was tired and led to the ward, was waiting for inpatients and majors.

The professor, who had a cup of coffee, often said, preventing me from hastily briefing and patient briefing because I wanted to finish the round quickly. "Ji Yong, why do you live like this?" So why do you have to see so many medical treatments? No professor, no patients, neither me.

Why is the medical treatment in Korea so malformed? It is a fact that the whole nation knows that all clinical settings, including internal medicine and orthopedics, are much shorter than those of developed countries. Still, can't you join the clinic for three minutes? Can you see a doctor in this short time? That was my idea at the time.

Occasional outpatient clinics were amazing. The technology of digging up information in a short time, the superhuman memory that contains the detail of numerous patients, and the patients who are getting amazingly improved in the short care. The professors also had a variety of mysterious techniques for cutting horses in no time.

Most impressive was the 'good bye' repetition, which continued until the patient got up from his seat. It looks easy, but it's a difficult technique. Keep a smile with a smile that will not offend your opponent, avoiding the eye of resentment that you haven't spoken enough. This technique, which I learned from squinting, is now used very often in my office.

After becoming a private business owner, I realized two things. The first is the practical reason that professors and seniors who had run the clinics before me had to do three minutes medical treatment. At the time of opening, I decided to make an appointment with the hot heart of the beginner, 'I will do the right treatment'. At least 20 minutes, if desired, 40 minutes to see the doctor. The patients were satisfied with me.

But at the end of each month, a dark future was drawn. The cost of psychiatric care was set by the state according to the time of consultation, but counseling was unthinkable. It was not the senior psychiatrist, but actually the decision of the state that the majority of the psychiatrists had only three minutes.

Fortunately, I still have a doctor's office on an appointment, and I have 40 minutes of consultation on some days. This is not because I am not particularly conscientious or lacking money. There are quite a few doctors these days, as well as me, and most of my motives do similar forms of care.

This change is also due to the state. In 2018, the government, which recognized the need to expand psychiatric care, increased the number, making it possible to hold long consultations with just over 10 people a day. Of course, such a day's income is much lower, but belief and reality can be compromised.

The second thing I noticed is that quite a few people want three minutes of medical care and are a very cost-effective form of care for them. There is no single cause of mental illness. Three of the biological, psychological, and environmental factors, such as changes in brain hormones, are affected. The weight of the three factors varies from person to person, so that some people may be able to change their environment, while others with large psychological problems do not improve medication. Another person's depression can only be resolved through medicine.

The environmental part is often beyond the reach of doctors, so focus on the other two factors. Therefore, the combination of medication and counseling is best, but for those who cannot afford both, medication alone is also a good alternative. In the case of diseases with a large proportion of biological factors, such as endogenous depression, panic disorder, and adult ADHD, only a short treatment and medications can improve well.

Frequently, people who have been disappointed with short medical treatments often ask 'Why not consult in psychiatry?' I have nothing to say. Because it was a fact. However, I would like to say that there are more options than you think. There are places that provide services such as medication, medication and counseling, psychoanalysis, and cognitive behavioral therapy. The menu, time and price are all different. Just as it takes to find a restaurant that fits your mouth, so does psychiatry.

So if you are wondering which psychiatry to go to, I would like to say that you should call at least two or three places to check the menu and prices. And if you're disappointed in one place, don't let go of the treatment for yourself. There must be a better place somewhere.
